What HMO, HMO-POS and PPO actually mean
Spokane County's 59 plans come in three network shapes. Understanding them is the fastest way to shrink your list.
- HMO (Health Maintenance Organization). You pick a primary care doctor and generally stay inside the plan's network, except for emergencies. Referrals may be required to see specialists.
- HMO-POS (Point of Service). Like an HMO, but with a limited "point of service" option to see some out-of-network providers, usually at a higher cost.

Special Needs Plans: 27 of the 59
Almost half of Spokane County's Medicare Advantage plans are Special Needs Plans. There are three flavors:
- D-SNP (Dual Eligible) — for people who have both Medicare and Medicaid. Examples on the county roster include Wellcare Dual Access (HMO-POS D-SNP), Community Health Plan of WA Dual Complete (HMO D-SNP), Wellpoint Dual Advantage (HMO D-SNP), Molina Medicare Complete Care (HMO D-SNP), and UHC Dual Complete WA-Q1 (PPO D-SNP), among others.

2027 rules that apply no matter which plan you pick
Some Medicare figures don't depend on which plan is on your card. For 2027:
- The Part D out-of-pocket cap is $2,100 in 2026 ($2,400 in 2027). Once you hit that in covered drug costs, your Part D plan pays 100% for the rest of the calendar year.
- The maximum Part D deductible is $615 in 2026 ($700 in 2027). Some plans set it lower.
- The old coverage gap ("donut hole") is gone as of 2025. Part D now has three phases: deductible, initial coverage, and catastrophic.
- The Medicare Prescription Payment Plan lets you spread your drug costs into predictable monthly bills through your Part D or MA-PD plan, at no extra charge.
- IRMAA surcharges (income-related extra premiums for Parts B and D) begin above $109,000 in income for a single filer in 2026.
