Wellcare's 2027 Medicare Advantage plans in Milwaukee: three plans, two renamed
MILWAUKEE — October 2026. As the Annual Enrollment Period opens, Wellcare is offering three Medicare Advantage plans to people with Medicare in Milwaukee County for 2027. Two of the three carry new names this year, and the carrier is not adding or dropping any plans in the county. If you are already in a Wellcare plan here, read the Annual Notice of Change (ANOC) that arrived in your mailbox — that document spells out what is different for January 1.

Key takeaways
- Wellcare offers three Medicare Advantage plans in Milwaukee County for 2027, all HMO-POS, all with drug coverage, and two are Dual Special Needs Plans (D-SNPs).
- No Wellcare plans are leaving the county for 2027 and none are brand-new; two were renamed, so the plan you had in 2026 may appear under a different name on Medicare's Plan Finder.
- The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027.
- Before you choose, confirm your doctors, hospitals and prescriptions against the plan's own 2027 directory and formulary at medicare.gov or by calling 1-800-MEDICARE.
MyMedicarePlans.org is an independent consumer site; we are not Wellcare, and we are not Medicare. Wellcare is one of seven carriers offering Medicare Advantage in Milwaukee County next year (52 plans in total across all carriers, per the CMS Medicare Advantage Landscape Source File for 2027).
What Wellcare offers in Milwaukee County for 2027
Wellcare's 2027 lineup here is three plans, all Health Maintenance Organization with a Point-of-Service option (HMO-POS):
- Wellcare MHS Health Wisconsin Dual Liberty Sync (HMO-POS D-SNP) — with drug coverage
- Wellcare MHS Health Wisconsin Dual Select (HMO-POS D-SNP) — with drug coverage
- Wellcare Simple (HMO-POS) — with drug coverage
The table below lists every Wellcare plan available in the county. Two of the three plans (the D-SNPs) are built for people who have both Medicare and Medicaid. The third is a general HMO-POS. All three include Part D prescription coverage, so you do not pair them with a separate drug plan.
What is new, and what is not returning
For 2027 in Milwaukee County, Wellcare has:
- 0 new plans
- 0 plans leaving
- 2 renamed plans
The renames: the plan previously called Wellcare Dual Access Sync (HMO-POS D-SNP) is now Wellcare MHS Health Wisconsin Dual Liberty Sync (HMO-POS D-SNP), and Wellcare Dual Reserve (HMO-POS D-SNP) is now Wellcare MHS Health Wisconsin Dual Select (HMO-POS D-SNP). A rename is not the same as "no changes" — the ANOC your plan mailed in late September is the document that lists what is actually different for 2027.
If you do nothing, your current plan rolls over into its 2027 version as long as the plan is still offered.
About Special Needs Plans
Two of Wellcare's three Milwaukee County plans are D-SNPs — Dual Special Needs Plans. These are only for people who qualify for both Medicare and Medicaid (in Wisconsin, that is Medicaid or certain Medicare Savings Programs). If you are not enrolled in Medicaid, you cannot join a D-SNP, even if you prefer the plan's features.
Wellcare does not offer a Chronic Condition SNP (C-SNP) or an Institutional SNP (I-SNP) in Milwaukee County for 2027.
How to check a plan against your doctors and prescriptions
Before December 7, do these four things for any plan you are considering — Wellcare or otherwise:
1. Go to medicare.gov/plan-compare. Enter your ZIP code and your medications (with dose and frequency). Plan Finder will show every plan available in Milwaukee County and estimate your yearly drug costs under each one. 2. Check the plan's provider directory for 2027. Networks change every year. The question is not just "is my doctor in the network today" but "are they in the 2027 network." The plan's website has a searchable directory; the plan's member services line can confirm. 3. Check the hospital. If you have a preferred hospital system in Milwaukee, verify it is in-network for the specific plan — not just the carrier generally. 4. Confirm the formulary. The drug list can change between plan years. Look up each of your prescriptions by name and dose and note the tier.
If anything is unclear, call 1-800-MEDICARE (TTY 1-877-486-2048), which is open 24 hours a day, seven days a week.
The 2027 numbers CMS has already published
Two Part D figures are confirmed for 2027:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026). Once your out-of-pocket spending on covered drugs hits that cap, you pay $0 for covered drugs the rest of the calendar year.
- The Part D maximum deductible is $700 in 2027 ($615 in 2026). Plans may charge less, but not more.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall. Be cautious of anyone quoting a 2027 Part B figure before CMS releases it.
