For 2027, Devoted Health is adding six new plans in Maricopa County:

One plan has a new name for 2027: DEVOTED C-SNP ENHANCED 014 AZ (HMO C-SNP) was previously called DEVOTED C-SNP PREMIUM 014 AZ. Same plan ID — new name. The ANOC will spell out any benefit changes that come with it.

No Devoted Health plans are leaving Maricopa County for 2027.

About Special Needs Plans

Six of Devoted's 2027 plans are Chronic Condition Special Needs Plans (C-SNPs). These are a type of Medicare Advantage plan built for people who have certain qualifying chronic conditions — such as diabetes, chronic heart failure, or chronic lung disorders.
