I have a Special Needs Plan now. Do I need to do anything for 2027?
Not necessarily. If your plan is returning and you still meet the eligibility rules (for a D-SNP, that usually means keeping your Medicaid status; for a C-SNP, having the qualifying chronic condition), it will renew January 1, 2027. Read your ANOC, and if your Medicaid status or health conditions have changed, call Humana or 1-800-MEDICARE to make sure you are still in the right plan.
My plan is not returning in 2027. What happens?
