A name change does not automatically mean the plan works the same way it did in 2026. Costs, drug lists and provider networks can shift from one plan year to the next. The Annual Notice of Change (ANOC) your plan mailed in late September is the document that tells you exactly what is different for 2027.
What are HMO-POS, PPO and D-SNP plans?
Wellcare's Fulton County lineup uses three plan structures:
- HMO-POS (Health Maintenance Organization with a Point-of-Service option) plans generally ask you to use an in-network primary care doctor and get referrals, with a limited ability to go out of network for certain services.
- PPO (Preferred Provider Organization) plans let you see providers outside the network, usually at a higher cost share, and typically do not require referrals.
- D-SNP (Dual-eligible Special Needs Plan) is a type of Medicare Advantage plan built for people who qualify for both Medicare and Medicaid. Enrollment is limited to people who meet those eligibility rules.

Three things to check before you enroll:
1. Your doctors and hospitals. Call the plan or search its online provider directory to confirm the specific providers you use are in network for the 2027 plan year. 2. Your prescriptions. Enter each drug and dose into Plan Finder to see whether it is on the plan's formulary, what tier it is on, and whether there are restrictions like prior authorization. 3. Your pharmacy. What is changing across Medicare itself for 2027?
A few program-wide numbers are already set by CMS for 2027:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026). Once your covered drug spending hits that cap, you pay $0 for covered Part D drugs the rest of the calendar year.
- The Part D maximum deductible is $700 in 2027 (up from $615 in 2026). Plans may charge less; this is the ceiling.
- The Part D national base beneficiary premium for 2027 is $41.33. This is a formula input CMS uses, not the premium any specific plan charges.
- The Medicare Prescription Payment Plan — an option that lets you spread Part D out-of-pocket costs across the year in monthly bills — continues in 2027.
The 2027 Part B standard premium and deductible and the 2027 IRMAA income brackets have not been announced yet. CMS typically publishes them later in the fall. We will update our coverage when those numbers are released.
What happens if I do nothing by December 7, 2026?
If you do nothing, your current plan renews for 2027 as long as it is still offered — with whatever changes are described in your Annual Notice of Change. None of Wellcare's Fulton County plans are being discontinued for 2027, so Wellcare members in Atlanta will roll over into the same plan (under its new name, for the four renamed D-SNPs).
If you want to switch Medicare Advantage plans, switch between Advantage and Original Medicare, or add or drop a Part D plan, the Annual Enrollment Period window is October 15 – December 7, 2026, and your change takes effect January 1, 2027. There is also a Medicare Advantage Open Enrollment Period from January 1 – March 31, 2027 that lets Advantage members make one more change.
