Key takeaways

  • Illinois has 139 Medicare Advantage plans for 2027 from 13 carriers, and you can compare the ones offered in your county on Medicare.gov.
  • All 139 plans include drug coverage for 2027, so most Illinois enrollees will not need a stand‑alone Part D plan alongside Medicare Advantage.
  • 23 plans are new for 2027 and 44 are not returning, which is why reading your current plan's Annual Notice of Change this fall matters.
  • The deadline to choose a 2027 plan is December 7, 2026, with coverage starting January 1, 2027.

Because Medicare Advantage is sold county by county, a plan listed statewide may or may not be offered where you live. The right way to compare is to look up the plans available at your own ZIP code and county, then line them up against your doctors, your prescriptions, and your pharmacies.

The statewide numbers for 2027

Across Illinois, the CMS Landscape file lists 139 Medicare Advantage plans for 2027 from 13 carriers. All 139 include prescription drug coverage. Year over year, 23 plans are new for 2027 and 44 are not returning. Those two figures are the reason current enrollees should not assume their 2026 plan will look the same, or still exist, in 2027.

Statewide totals describe what is on the shelf across Illinois. What matters to you is the shorter list on the shelf in your own county, which you can pull up on the Medicare Plan Finder at Medicare.gov.

Plan types and Special Needs Plans

The 139 plans break down by type as follows: 44 HMO, 24 HMO‑POS, 66 PPO, 2 PFFS, and 3 Cost plans. HMO and HMO‑POS plans generally focus care around an in‑network set of providers; PPO plans typically allow some out‑of‑network use; PFFS and Cost plans follow different rules again. The Evidence of Coverage for any specific plan spells out how its rules actually work.

Illinois also has Special Needs Plans for 2027: 4 D‑SNPs (for people with both Medicare and Medicaid), 23 C‑SNPs (for people with qualifying chronic conditions), and 3 I‑SNPs (for people living in or needing an institutional level of care). SNPs have eligibility rules, so not everyone can enroll in one; 1‑800‑MEDICARE or the plan itself can confirm whether you qualify.

The carriers offering plans in Illinois for 2027

Thirteen carriers are on the 2027 Illinois landscape. In alphabetical order, with their statewide plan counts, they are: Aetna (CVS Health) with 22 plans; Blue Cross Blue Shield (HCSC) with 2; Centene (WellCare) with 7; Devoted Health with 24; Essence Healthcare with 9; HealthSpring with 6; Humana with 32; Longevity Health with 1; Medical Associates Health Plan, Inc. with 3; Molina Healthcare of Illinois with 1; Provider Partners Health Plans with 1; UnitedHealthcare with 24; and Zing Health with 7. The full carrier and plan tables appear on this page.

A larger carrier count does not mean a carrier is a better fit, and a smaller count does not mean a carrier is a worse fit. It only tells you how many distinct plans the carrier filed with CMS statewide. What matters is which of those plans is actually offered in your county and how it handles the care you use.

How to compare the plans in your own county

A clear, repeatable way to narrow 139 statewide plans down to the one that fits you:

1. Pull your county's list. Go to the Medicare Plan Finder at Medicare.gov and enter your ZIP code. You will see only the Medicare Advantage plans available where you live. 2. Check your doctors and hospitals. For each plan you are considering, confirm that your primary care doctor, specialists, and preferred hospital are in network. Verify on the plan's own provider directory and, if it matters, call the office. 3. Enter your prescriptions. Add every drug you take, with dose and quantity, into Plan Finder. Each plan's formulary and cost‑sharing is different, and this is where surprises usually show up. 4. Check your pharmacy. The same drug can be priced differently at different pharmacies within the same plan. Compare the pharmacy you actually use. 5. Read your Annual Notice of Change (ANOC). If you are already in a Medicare Advantage plan, your carrier mailed an ANOC in late September describing what is changing for 2027. Read it before you decide whether to stay or switch. 6. Confirm plan type and rules. HMO, HMO‑POS, PPO, PFFS, Cost, and SNP plans each have different referral, network, and eligibility rules. The Evidence of Coverage is the authoritative document. 7. Write it down. Keep a short side‑by‑side of two or three finalists so you can compare on the same terms rather than from memory.

What to do before December 7

Mark December 7, 2026 on the calendar as the last day of the Annual Enrollment Period for 2027 coverage. Between October 15 and December 7, you can enroll in a Medicare Advantage plan, switch from one Medicare Advantage plan to another, drop Medicare Advantage and return to Original Medicare, or add or change a Part D plan where applicable. Whatever you choose during that window starts January 1, 2027.

If your current plan is among the 44 not returning for 2027, you will get a non‑renewal notice from the carrier, and you will have a Special Enrollment Period in addition to AEP. If you are happy with your current plan, read the ANOC, confirm your doctors and drugs are still covered the way you expect, and you do not need to do anything to stay.
