FORT WAYNE, Ind. — Fall 2026. As the Annual Enrollment Period gets under way, Allen County residents living with certain chronic conditions have a small but distinct slice of the Medicare Advantage lineup to consider: Chronic Condition Special Needs Plans, or C-SNPs. For plan year 2027, five of the 64 Medicare Advantage plans offered in the county are C-SNPs, built around a qualifying diagnosis a doctor confirms.
These plans are not for everyone — that is the point. They are designed for people who already live with a specific condition, and enrollment requires paperwork from the provider who treats it.
What is a C-SNP, and how does one work in Fort Wayne?
A Chronic Condition Special Needs Plan is a type of Medicare Advantage plan limited to people with a specific chronic illness, such as diabetes, chronic heart failure or chronic kidney disease. In Allen County, where Fort Wayne sits, 5 C-SNPs are on the 2027 lineup. Like other Medicare Advantage plans, a C-SNP replaces Original Medicare and usually includes Part D drug coverage. The difference is the design: the drug list, care coordination and provider network are shaped around the condition the plan serves.
Because the plan is tailored, it is only open to people who actually have that condition. CMS requires a provider to verify the diagnosis before enrollment is final. If the diagnosis cannot be confirmed within a set window after you sign up, the plan has to disenroll you.
Which C-SNPs are offered in Allen County for 2027?
Five C-SNPs are available to Allen County residents for 2027, from three carriers. Listed alphabetically by carrier, they are:
- Devoted Health — DEVOTED C-SNP CHOICE ENHANCED 015 IN (PPO C-SNP); DEVOTED C-SNP CHOICE GIVEBACK EXTRAS 019 IN (PPO C-SNP); DEVOTED C-SNP CHOICE PLUS 013 IN (PPO C-SNP). All three are new for 2027.
- Humana — Humana Gold Plus - Diabetes and Heart (HMO C-SNP).
- UnitedHealthcare — UHC Complete Care IN-21 (HMO-POS C-SNP).
The table on this page lists each plan with its type and whether it includes drug coverage. We are not quoting premiums, copays, networks or star ratings here — those details change by plan and belong on the Medicare Plan Finder at medicare.gov. The Humana plan names the conditions it serves; for the others, the condition each plan covers is set in CMS filings and listed in the plan's materials.
For context, C-SNPs are one of three Special Needs Plan types. Allen County's 2027 lineup also includes 12 Dual Eligible SNPs (D-SNPs), for people with both Medicare and Medicaid, and 8 Institutional SNPs (I-SNPs), for people in nursing facilities or needing that level of care.
Who qualifies to enroll in a C-SNP?
You qualify if a provider confirms you have the chronic condition the plan is designed for. That is the gatekeeper. CMS maintains a list of qualifying conditions — diabetes, chronic heart failure, chronic lung disorders, end-stage renal disease and cardiovascular disorders are among them — and each C-SNP targets one or a small cluster of them.
When you apply, the plan sends a short form to the doctor or clinician treating the condition. They verify the diagnosis in writing. Without that verification, the enrollment does not stick. If your condition changes or improves, the plan has rules about how long you can stay enrolled.
When can I sign up for a C-SNP?
Two windows matter. The first is the Annual Enrollment Period, Oct. 15 – Dec. 7, 2026, when anyone with Medicare can join, switch or drop a Medicare Advantage plan for coverage starting Jan. 1, 2027. The second is a Special Enrollment Period tied to the qualifying diagnosis itself: people with a qualifying chronic condition can enroll in a C-SNP once, outside the usual windows, when they first become eligible for that plan. There is also a Special Enrollment Period to leave the C-SNP if the condition no longer applies.
That means you do not have to rush a decision before Dec. 7 just because you have a qualifying condition. If a C-SNP fits later in 2027, the SEP is there.
What is changing in Allen County's 2027 lineup overall?
The Medicare Advantage menu in Allen County for 2027 includes 64 plans. Compared to 2026, 10 plans are not returning, 16 are new, and 6 have been renamed. If you are already in a Medicare Advantage plan — C-SNP or otherwise — your plan's Annual Notice of Change (ANOC), mailed in late September, lists exactly what is different for 2027: cost changes, drug list changes, network changes. If you do nothing and your plan is still offered, it renews automatically on Jan. 1, 2027.
A few program-wide numbers for Part D are already set for 2027: the out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the maximum Part D deductible is $700 (up from $615). The 2027 Part B standard premium and deductible, and the 2027 IRMAA brackets, had not been announced as of early fall 2026; CMS typically publishes them in November.
