Chula Vista, Calif. — As the Medicare Annual Enrollment Period opens for 2027 coverage, people living with a chronic illness in Chula Vista have a specific type of Medicare Advantage plan to consider: a Chronic Condition Special Needs Plan, or C-SNP. For 2027, San Diego County — where Chula Vista sits — will have 32 C-SNPs on the market, up from the mix offered this year, with new entries from several carriers and a handful of renamed plans.
C-SNPs are not for everyone. To join one, a person has to have a qualifying chronic condition that a doctor or other provider confirms in writing. The payoff is a plan built around that condition, with a drug list and care coordination designed for people who share it.
The Annual Enrollment Period runs October 15 through December 7, 2026. Changes made in that window take effect January 1, 2027.
What is a C-SNP, and how does it work in Chula Vista for 2027?
A C-SNP is a type of Medicare Advantage plan that only enrolls people who have a specific chronic health condition. In San Diego County, where Chula Vista is located, 32 of the 85 Medicare Advantage plans available for 2027 are C-SNPs.
Like other Medicare Advantage plans, a C-SNP covers everything Original Medicare covers, and almost all of them include Part D drug coverage. What makes a C-SNP different is focus: the plan's provider network, drug list and care management are shaped around one condition (or a group of related conditions, such as diabetes and heart disease). Members usually get a care coordinator who helps schedule appointments, track medications and connect the different providers involved.
Because these plans are built for a defined population, Medicare limits who can join.
Who qualifies to enroll in a C-SNP?
Only people with a qualifying chronic condition can enroll, and that condition has to be verified by a provider. When you apply, the plan will ask for the name of the doctor who diagnosed and treats the condition, then contact that provider to confirm. If the provider does not confirm the diagnosis within the plan's time limit, the enrollment can be cancelled.
Medicare sets the list of conditions that qualify. It includes things like diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders such as COPD, end-stage renal disease, and several others. A plan may be approved for one condition or for a group of related ones — the plan's materials spell out exactly which diagnoses it accepts.
If you aren't sure whether your condition qualifies, your doctor's office or 1-800-MEDICARE can tell you.
Which carriers offer C-SNPs in San Diego County for 2027?
Eleven carriers will offer C-SNPs in San Diego County for 2027. Listed alphabetically, they are Aetna (CVS Health), Alignment Health Plan, Astiva Health, Centene (WellCare), Champion Heath Plan, Clever Care Health Plan, Community Health Group, Elevance Health (Anthem), Imperial Health Plan of California, Inc., SCAN Health Plan and UnitedHealthcare.
The full table below lists every C-SNP available in the county, by carrier. A few notes on what is changing year to year:
- New for 2027: Aetna Medicare Chronic Care, Astiva Health C-SNP Bloom, Astiva Health C-SNP Fountain, Champion Compass, Champion Home, Champion Secure, Clever Care Breathe Select+, Clever Care Total Choice+, Anthem Kidney Care, and Imperial Flex are entering the market.
- Renamed: Several Alignment Health C-SNPs — including the BreathEasy Plus, Clarity Plus, and Heart & Diabetes lines — carry new names for 2027, as do UHC Sharp Complete Care CA-15P and UHC Sharp Complete Care Support CA-6AP.
Only the plan names themselves tell you what condition a plan serves — for example, "Heart & Diabetes," "BreathEasy" or "Kidney Care." For any plan whose name is more general, call the plan or check Medicare.gov to see which diagnoses it accepts.
When can I join a C-SNP outside the fall enrollment window?
People who qualify for a C-SNP get a Special Enrollment Period tied to their diagnosis. In practice, that means someone newly diagnosed with a qualifying condition does not have to wait for October 15 to make a change — they can enroll in a C-SNP once during the time between the diagnosis and when they are enrolled in that type of plan.
The Special Enrollment Period also works the other way. If a person is on a C-SNP and no longer has the qualifying condition, they'll generally be moved out of the plan and get a window to pick a different Medicare Advantage or Original Medicare option.
Outside of these condition-based rules, the standard windows still apply: the Annual Enrollment Period (October 15 – December 7) and the Medicare Advantage Open Enrollment Period (January 1 – March 31), which lets someone already in a Medicare Advantage plan switch once.
What else is changing for 2027 that C-SNP shoppers should know?
A few program-level numbers are already set for 2027 and affect anyone with Part D drug coverage — which includes almost every C-SNP:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (up from $615 in 2026).
- The coverage gap, or "donut hole," remains eliminated; Part D now has three phases.
- The Medicare Prescription Payment Plan remains available as an option to spread drug costs across the year in level monthly amounts.
The 2027 Part B standard premium, deductible and IRMAA income brackets have not yet been announced; CMS typically publishes them later in the fall.
Across San Diego County as a whole, 26 Medicare Advantage plans are not returning for 2027, 18 are new, and 20 have been renamed. If your current plan is one of them, the Annual Notice of Change (ANOC) that arrives in the mail each fall explains exactly what is different.
What should I do if I think a C-SNP fits my situation?
Start with your doctor. Ask whether your diagnosis is one Medicare treats as a qualifying chronic condition for a C-SNP, and whether their office is in the network of any C-SNPs offered in San Diego County.
Then compare plans on Medicare.gov using the Plan Finder tool, which shows each plan's premium, drug list, pharmacy network and provider directory for your specific prescriptions and doctors. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, seven days a week.
If you take no action, your current plan renews for 2027 with the changes described in its ANOC. There is no penalty for staying put — but reading the ANOC is worth the time either way.
