Who qualifies to enroll in a C-SNP?
To enroll in a C-SNP, you must have Medicare Part A and Part B, live in the plan's service area, and have the specific chronic condition the plan is designed for. That diagnosis has to be confirmed by a qualified provider — not just self-reported. The plan will typically send a short form for your doctor to sign; if the diagnosis is not verified within the time CMS allows, enrollment can be ended.
This is the single most important thing to understand about C-SNPs. A regular Medicare Advantage plan does not require a diagnosis to join. A C-SNP does.
When can you sign up for a C-SNP in Santa Ana?
