IRVINE, Calif. — As the Medicare Annual Enrollment Period opens on Oct. 15, 2026, people in Irvine who live with a serious chronic condition — things like diabetes, heart failure, chronic lung disease or end-stage renal disease — have a specific kind of Medicare Advantage plan built for them. For 2027, Orange County will have 41 Chronic Condition Special Needs Plans, known as C-SNPs, from 11 carriers. These plans are a slice of the county's 111 Medicare Advantage plans.
If a doctor has confirmed one of the qualifying conditions, a C-SNP may be worth a look. And unlike most Medicare choices, you do not have to wait for the fall window to join one.
What is a Chronic Condition Special Needs Plan (C-SNP)?
A C-SNP is a type of Medicare Advantage plan that only accepts people living with one or more specific chronic illnesses. Medicare limits C-SNPs to a defined list of conditions — for example, diabetes, chronic heart failure, cardiovascular disorders, chronic lung disorders, end-stage renal disease and a handful of others.
Because every member shares the same condition, the plan can shape its drug list, provider network and care coordination around that illness. Most C-SNPs include Part D prescription drug coverage, and all of the 2027 C-SNPs in Orange County do. You still get everything Original Medicare covers; it just comes through the plan.

Who qualifies for a C-SNP in Irvine, and how is the diagnosis confirmed?
To join a C-SNP, you need a qualifying chronic condition and a provider has to confirm it. In practice, the plan sends a short form to the doctor you name during enrollment. The doctor returns it to verify the diagnosis. If verification does not come back within the window Medicare allows, the enrollment cannot stand.
A plan's name sometimes tells you which condition it serves — "Heart & Diabetes," "Kidney Care," "Lung Care," "Dialysis Care" — but many C-SNPs cover more than one condition, and names do not spell out every eligibility rule. Before you apply, call the plan or check the Medicare Plan Finder at medicare.gov to confirm the plan matches your diagnosis.
When can someone with a qualifying condition enroll in a C-SNP?
People with a qualifying chronic condition get a Special Enrollment Period (SEP) that lets them join a C-SNP once at any time of the year, outside the usual windows. That SEP is one of the main reasons C-SNPs exist as a separate category.
The regular windows still apply too: the Annual Enrollment Period runs Oct. 15 through Dec. 7, 2026, with coverage starting Jan. 1, 2027. The Medicare Advantage Open Enrollment Period runs Jan. 1 – March 31, 2027. If you are new to Medicare, your Initial Enrollment Period around your 65th birthday is the first door in.
