A good step-by-step before December 7, 2026:

1. Enter your ZIP code and your drug list on medicare.gov. The Plan Finder will show every Medicare Advantage plan available in Orleans Parish, including the Anthem D-SNPs if you qualify. 2. Open the plan details and check the provider directory for your primary care doctor, your specialists and the hospital you prefer. HMO plans generally require you to use in-network providers except in emergencies. 3. Confirm each of your prescriptions is on the plan's formulary, and note the tier and any prior-authorization or step-therapy rules. 4. What happens if I do nothing by December 7, 2026?

If you do nothing, your current plan renews on January 1, 2027, with the changes described in its Annual Notice of Change. For most people that is fine; for some, a change in formulary, network or cost-sharing is enough reason to shop around.

You also get a second window: the Medicare Advantage Open Enrollment Period, January 1 through March 31, 2027. During that window, if you are already in a Medicare Advantage plan, you can switch to a different Medicare Advantage plan or go back to Original Medicare (and add a Part D plan). You cannot use that window to jump from Original Medicare into a Medicare Advantage plan for the first time.
