ST. LOUIS — As the Annual Enrollment Period opens Oct. 15, 2026, the Medicare Advantage lineup for St. Louis city is shifting around the edges. Eleven carriers plan to sell Advantage coverage here for 2027, and the mix includes a handful of new plans, several name changes, and ten plans that will not come back next year.

If you live in St. Louis, this is the moment to open your plan's Annual Notice of Change (ANOC) and see where your plan sits in that shuffle before the Dec. 7, 2026 deadline.

Which carriers sell Medicare Advantage in St. Louis for 2026, and how is the 2027 lineup shaping up?

St. Louis is in St. Louis city, where CMS lists 79 Medicare Advantage plans from 11 carriers for 2026. By plan type, that breaks down to 31 HMO plans, 20 HMO-POS plans, 27 PPO plans and 1 PFFS plan. Of those, 34 are Special Needs Plans (SNPs) and 72 include Part D drug coverage.

For 2027, CMS's year-over-year comparison shows 10 plans leaving, 11 plans new, 17 renamed, and 52 continuing unchanged in name and basic structure. Another 24 continuing plans show a change on premium, deductible, type or name — the kind of detail you will see spelled out in your ANOC.

The 11 carriers offering plans in St. Louis city for 2026 are Aetna (CVS Health), American Health Advantage of Missouri, Blue Cross Blue Shield (HCSC), Centene (WellCare), Devoted Health, Elevance Health (Anthem), Essence Healthcare, Humana, NHC Advantage, Provider Partners Health Plans, and UnitedHealthcare. The full plan-by-plan roster is in the table below.

Which Medicare Advantage plans are not returning to St. Louis in 2027?

Ten plans will not be offered in St. Louis city for 2027. They are:

A note on the Provider Partners names: two plans in the "leaving" list share names with plans in the "new for 2027" list. CMS treats them as different contracts, so if you are enrolled in one of these today, read your ANOC carefully to see which contract is ending and which is replacing it.

Which Medicare Advantage plans are new in St. Louis for 2027?

Eleven plans are new to St. Louis city for the 2027 plan year:

Several of these are Special Needs Plans, meaning you have to qualify — for example, by having both Medicare and Medicaid (D-SNP), by living in a nursing facility (I-SNP), or by having a listed chronic condition (C-SNP).

Which St. Louis Medicare Advantage plans were renamed for 2027?

Seventeen plans in St. Louis city carry a new name for 2027 but are the same contract. A rename does not, by itself, mean benefits are changing — but it is a strong signal to open the ANOC and look. Among the renames: three Aetna PPOs (Aetna Medicare Elite Classic, Aetna Medicare Elite Classic Extra and Aetna Medicare Enhanced Classic); three Wellcare D-SNPs now carrying "Home State Health" in the name; one Devoted C-SNP (DEVOTED C-SNP ENHANCED 015 MO); several Humana plans now sold as Humana Essentials Plus Giveback, Humana Full Access, Humana Total Complete and Humana Value Choice; and two UnitedHealthcare D-SNPs renumbered as UHC Dual Complete MO-Q1 and MO-Q2.

What happens if my St. Louis Medicare Advantage plan is leaving in 2027?

If your plan is on the "leaving" list, you will get a non-renewal notice from your carrier. You then qualify for a Special Enrollment Period that lets you pick a different Medicare Advantage plan or go back to Original Medicare with a stand-alone Part D drug plan — outside the regular Annual Enrollment calendar. Medicare.gov and 1-800-MEDICARE can walk you through the exact dates for your situation.

If you do nothing and your plan is still being offered next year, it simply renews on Jan. 1, 2027 with the changes described in its ANOC.

What are the key 2027 Medicare dates and dollar figures to know?

The Annual Enrollment Period for 2027 coverage runs Oct. 15 – Dec. 7, 2026. The Medicare Advantage Open Enrollment Period, for people already in an Advantage plan who want to switch once, runs Jan. 1 – March 31, 2027.

On the Part D side, CMS has confirmed a few 2027 figures:

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet; CMS typically publishes them later in the fall. The Medicare Prescription Payment Plan, which lets you spread drug costs over the year, is still available.

Frequently asked questions

When do I need to decide on my 2027 Medicare coverage?
The Annual Enrollment Period runs Oct. 15 – Dec. 7, 2026. Any change you make in that window takes effect Jan. 1, 2027. If you do nothing and your plan is being offered next year, it renews automatically with the changes listed in your Annual Notice of Change.
My Medicare Advantage plan is on the "leaving" list for St. Louis. What are my options?
You will get a non-renewal letter from your carrier. You can choose a different Medicare Advantage plan available in St. Louis city for 2027, or return to Original Medicare and add a stand-alone Part D plan. Because your plan is being discontinued, you qualify for a Special Enrollment Period that extends beyond Dec. 7, 2026 — the exact dates are explained in your notice and at 1-800-MEDICARE.
My plan has a new name for 2027 — did my coverage change?
Not necessarily. A rename alone does not change what the plan covers, but carriers often rename a plan when something else is being updated too. Read the Annual Notice of Change your carrier sends in September or October; it spells out any change to the premium, deductible, drug list, network or extra benefits.
Where can I compare the full 2027 St. Louis lineup side by side?
The official Medicare Plan Finder at medicare.gov lets you enter your ZIP code and your prescriptions and see every plan available to you, with the 2027 details. You can also call 1-800-MEDICARE, 24 hours a day, seven days a week, for help in your own language. For personalized answers about your coverage, your prescriptions and your providers, start at medicare.gov or call 1-800-MEDICARE (1-800-633-4227), TTY 1-877-486-2048.
