What Humana Medicare Advantage plans are available in Cleveland for 2027?
Humana will offer 21 Medicare Advantage plans in Cuyahoga County, the county Cleveland sits in, for the 2027 plan year. By type, that breaks down to 2 HMO plans, 3 HMO-POS plans, and 16 PPO plans. Seventeen of the 21 plans include Part D prescription drug coverage; the others are medical-only.
The full roster — every plan name, plan type, and whether drugs are included — is in the table below. CMS publishes this list in the Medicare Advantage Landscape Source File each fall.
Which Humana plans are new, renamed, or not returning in Cleveland for 2027?
In Cuyahoga County, Humana has five Medicare Advantage plans new for 2027, one plan that has been renamed, and nine 2026 plans that are not being offered again next year.
The new names on the 2027 list are Humana Essentials Plus Giveback (PPO), Humana Gold Plus H0292-004 (HMO-POS), Humana Senior Living Plan (PPO I-SNP), a second Humana Together in Health (PPO I-SNP) offering, and HumanaChoice Giveback H5216-481 (PPO). The renamed plan is Humana Full Access (PPO), which was listed in 2026 as Humana Full Access H5525-042 (PPO).
If you are currently in a Humana plan that is not on the 2027 list, your ANOC should tell you so and explain your options. You can also check your plan status at medicare.gov or by calling 1-800-MEDICARE.
Which Humana Special Needs Plans are offered in Cuyahoga County for 2027?
Humana's 2027 Cuyahoga County lineup includes 4 Special Needs Plans: 1 Chronic Condition SNP (C-SNP) and 3 Institutional SNPs (I-SNP). There are no Humana D-SNPs on the 2027 county list.
Special Needs Plans are limited to people who meet specific eligibility rules — a qualifying chronic condition for a C-SNP, or living in a long-term care setting (or needing that level of care) for an I-SNP. Medicare.gov can confirm whether you qualify before you enroll.
How do I check a Humana plan against my doctors and prescriptions before December 7?
Use the Medicare Plan Finder at medicare.gov. Enter your ZIP code and your prescriptions, then open any plan to see its provider network, formulary (drug list), copays, premium, and star rating. You can also call 1-800-MEDICARE, 24 hours a day, for the same information read aloud. For the most official answer on a specific plan, request the Evidence of Coverage and the provider and pharmacy directories from Humana directly.
Three quick checks worth doing every year, even if you plan to stay put:
- Confirm each doctor and hospital you use is still in-network for 2027.
- Confirm each prescription is on the 2027 formulary and note the tier.
- Read the ANOC your plan mailed in late September for changes to premiums, copays, and extra benefits.
What is changing in Medicare drug coverage for 2027?
Two Part D numbers are already confirmed for 2027. The annual out-of-pocket cap on covered prescriptions rises to $2,400 in 2027, up from $2,100 in 2026. The maximum Part D deductible a plan can charge rises to $700 in 2027, up from $615 in 2026. The Medicare Prescription Payment Plan — the option to spread drug costs across the year in monthly bills — continues.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been announced as of September 2026. CMS typically publishes those figures later in the fall.
What happens on December 7, and what if I do nothing?
December 7, 2026 is the last day of the Annual Enrollment Period for 2027 coverage. A plan change you make between October 15 and December 7 takes effect January 1, 2027.
If you do nothing and your current plan is on the 2027 list, you stay enrolled and the changes in your ANOC take effect automatically. If your plan is one of the nine Humana plans not returning, you will need to pick another plan or you may be moved back to Original Medicare. Medicare Advantage members also get a second window — Medicare Advantage Open Enrollment, January 1 through March 31, 2027 — to make one change.
