Special Needs Plans in the lineup
Of UnitedHealthcare's 12 plans in Franklin County next year, 4 are Special Needs Plans — Medicare Advantage plans designed for people in a specific situation. They include:
- Two C-SNPs (Chronic Condition SNPs): UHC Complete Care OH-18 and UHC Complete Care Support OH-1A. These are for people living with certain qualifying chronic conditions.
- Two I-SNPs (Institutional SNPs): UHC Care Advantage OH-E001 and UHC Nursing Home Plan OH-F001. These are for people who live in, or need the level of care provided in, a nursing home or similar setting.
For 2027, UnitedHealthcare is not offering a D-SNP (for people with both Medicare and Medicaid) in Franklin County under its own carrier listing. If you were enrolled in a D-SNP for 2026, read the Annual Notice of Change your plan mails you in September so you know what to do next.

Nothing happens to anyone who does nothing: if your current plan is offered again next year, it rolls over on January 1 with the updates in that ANOC packet.
How to check a plan against your doctors and prescriptions
Before December 7, it is worth taking 20 minutes to confirm two things for any plan you are considering:
1. Your doctors and hospitals. Call the plan directly or check its online provider directory to confirm each of your providers is in network for 2027. Networks change year to year. 2. Your prescriptions. Use the Medicare Plan Finder at medicare.gov. Enter each drug, dose and pharmacy, and the tool will show you how each plan covers them, including the plan's formulary tier and any restrictions.
A few 2027 Part D numbers to keep in mind as you compare drug coverage:
- The Part D out-of-pocket cap is $2,400 for 2027, up from $2,100 in 2026.
- The Part D maximum deductible is $700 for 2027, up from $615 in 2026.
- The coverage gap (the old "donut hole") was eliminated in 2025; Part D now has three phases.
- If drug costs hit hard in any one month, the Medicare Prescription Payment Plan lets you spread what you owe across the rest of the year, interest-free.
CMS has not yet published the 2027 Part B standard premium and deductible, or the 2027 IRMAA income brackets. Those usually come out in November — check medicare.gov once they are announced.
