PHILADELPHIA — Fall 2026 is here, and Medicare's Annual Enrollment Period is open through December 7, 2026. Anything you change in that window takes effect January 1, 2027. If you do nothing, your current Medicare Advantage plan simply renews — but it may not look the same next year, which is why a careful side-by-side is worth an hour of your time.

Philadelphia is in Philadelphia County, where the 2027 Medicare Advantage lineup is set. According to the CMS Medicare Advantage Landscape Source File, the county will have 98 Medicare Advantage plans from 17 carriers for 2027. All 98 include prescription drug coverage. There are 13 new plans and 12 plans not returning, and plan types break down as 47 HMO, 17 HMO-POS and 34 PPO. Start with the Annual Notice of Change

Every Medicare Advantage and Part D plan mails an Annual Notice of Change (ANOC) by late September. It compares your 2026 plan to the 2027 version — premium, deductible, copays, drug tier changes, network updates and extra benefits. Read it before you shop. If nothing important changed and you are still happy, you may be fine to let your plan renew. If something you rely on changed, that is your signal to compare.

For context on program-wide drug costs: in 2027, the Part D out-of-pocket cap rises to $2,400 (from $2,100 in 2026), and the Part D maximum deductible is $700 (from $615 in 2026). The 2027 Part B standard premium, deductible and IRMAA income brackets have not been announced yet; CMS typically publishes those in the fall.

A step-by-step way to compare

Open the Medicare Plan Finder at medicare.gov/plan-compare and sign in (or use it as a guest). Then walk through these checks for every plan on your short list:

1. Doctors. Enter the names of each doctor and specialist you want to keep. Confirm they are in-network for the specific 2027 plan — not just the carrier. 2. Hospitals. Check that your preferred Philadelphia-area hospital system is in-network. Networks can differ between a carrier's HMO and PPO. 3. Prescriptions. Enter every drug, exact dose and quantity. The Plan Finder will show the plan's 2027 formulary tier, any restrictions (prior authorization, step therapy) and your estimated yearly drug costs. 4. Pharmacies. Pick 1–2 pharmacies you actually use. "Preferred" pharmacies often cost less than "standard" ones at the same plan. 5. Total cost, not just premium. Look at the plan's premium, deductible, copays and the yearly out-of-pocket maximum together. 6. Extra benefits. If dental, vision, hearing, OTC or fitness matter to you, read the plan documents (not just the summary) to see what is actually covered. 7. Star Ratings. CMS rates plans 1–5 stars on quality and member experience. Use them as one data point among many.

What's new for 2027 in the county

The 2027 Philadelphia County roster adds 13 new plans and drops 12 plans from 2026. If one of the 12 not-returning plans is yours, you will get a non-renewal notice from your carrier and will need to pick a new plan during AEP (or you'll have a Special Enrollment Period after). Some plans have also been renamed — the ANOC spells out whether yours is one of them.

Plan-type mix also shifts year over year. For 2027, HMOs grew to 47 and HMO-POS plans came down to 17; PPOs stand at 34. The Special Needs Plan count is 20 D-SNP, 8 C-SNP and 7 I-SNP. If you do nothing

You will not lose coverage simply by sitting still, as long as your plan is offered again in 2027. It will renew with the changes described in your ANOC. The risk of doing nothing isn't losing Medicare — it's being surprised in January by a copay, a drug tier or a dropped provider you didn't know had changed.
