Before December 7: vetting your Nashville Humana plan
NASHVILLE — As the 2027 Annual Enrollment Period runs from October 15 through December 7, 2026, Humana's Medicare Advantage lineup in Davidson County is shifting: the carrier is offering 11 plans here next year, with 1 new plan, 5 plans not returning, and 3 renamed. If you already have a Humana plan, nothing happens automatically beyond the changes spelled out in your Annual Notice of Change (ANOC) — but this is the window to check that your doctors, hospitals and prescriptions still line up with the plan you'll have in January.

What Humana Medicare Advantage plans are offered in Nashville for 2027?
In Davidson County, which is the county Nashville sits in, Humana is offering 11 Medicare Advantage plans for 2027. By type, that breaks down to 5 HMOs and 6 PPOs. Four of the 11 are Special Needs Plans — 2 Dual-eligible SNPs (D-SNPs) for people with both Medicare and Medicaid, and 2 Institutional SNPs (I-SNPs) for people who live in or need the level of care typically provided in a long-term care facility. Humana is not offering a Chronic Condition SNP (C-SNP) in the county next year.
Eight of the 11 plans include Part D prescription drug coverage built in. Three do not — if you choose one of those and want drug coverage, you would generally need to think about how you'll handle prescriptions. The table on this page lists every Humana plan available in the county for 2027, by name and type.
What's new, renamed or not returning in Humana's 2027 Nashville lineup?
Compared with 2026, Humana has 1 new plan in Davidson County for 2027, 5 plans that are not returning, and 3 plans that kept the same underlying plan but got a new name on the paperwork.
The renames to watch for on your mail: the plan previously called Humana Gold Plus SNP-DE H4461-038 (HMO D-SNP) is now listed as Humana Dual QMB Only (HMO D-SNP); Humana Essentials Plus Giveback H4461-039 (HMO) is now Humana Essentials Plus Giveback (HMO); and Humana Total Complete H4461-043 (HMO) is now Humana Total Complete (HMO). A rename alone doesn't tell you whether the benefits changed — the ANOC Humana mailed in September does.
If your 2026 plan is one of the five not returning for 2027, Humana will have sent you a non-renewal notice. You have a Special Enrollment Period to choose a new plan, but you don't have to stay with the same carrier.
What happens if I do nothing before December 7, 2026?
If your Humana plan is being offered again in 2027, it renews automatically on January 1, 2027, with whatever changes are described in your ANOC. You don't need to do anything to keep it.
What you do want to do before December 7 is read that ANOC. It lists any changes to the monthly premium, the deductible, the drug list (formulary), the pharmacy network, the provider network, and extra benefits. Those are the pieces most likely to affect you in January.
How do I check a Humana plan against my doctors and prescriptions?
The most reliable way is the Medicare Plan Finder at medicare.gov. You can enter your ZIP code, type in your prescriptions and preferred pharmacy, and the tool will show you each plan's estimated yearly drug costs and whether your pharmacy is in network. For doctors and hospitals, check the plan's own provider directory on Humana's site, or call the plan directly using the member services number on the plan's page — networks can change year to year, so a provider who was in network in 2026 may or may not be in 2027.
A few things worth confirming before you commit:
- Each prescription you take is on the 2027 formulary, and at what tier.
- Your primary care doctor and any specialists you see regularly are in the 2027 network.
- The hospital you'd want to use is in network.
- Any routine services you rely on (for example, a specific pharmacy, dialysis center or home health agency) are covered the way you expect.
If you'd rather talk to a person, 1-800-MEDICARE (1-800-633-4227) is open 24/7 and can walk you through plan options for your ZIP code.
What 2027 Medicare numbers are already confirmed?
A few program-level figures for 2027 are already set by CMS. The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026). The Part D maximum deductible a plan can charge rises to $700 in 2027 (up from $615 in 2026). The Part D national base beneficiary premium for 2027 is $41.33 — that's a formula input CMS uses, not what any particular plan charges.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those later in the fall.
The coverage gap (the old "donut hole") was eliminated in 2025, so Part D now has three phases: deductible, initial coverage and catastrophic. The Medicare Prescription Payment Plan — the option to spread your drug costs across the year in monthly installments — is still available in 2027 at no extra charge if you opt in.
