HMO, HMO-POS and PPO plans treat out-of-network care differently. 2. Prescriptions. Check that each medication you take is on the plan's formulary, and look at the tier and any coverage rules such as prior authorization or step therapy. 3. Extra benefits and rules. Review the plan's Summary of Benefits and Evidence of Coverage for how the plan handles referrals, out-of-network care and any added benefits.
