What Humana offers in Los Angeles County for 2027
For plan year 2027, Humana has 8 Medicare Advantage plans available to Los Angeles County residents. By type, that breaks down to 4 HMO plans and 4 PPO plans. All 8 include Part D prescription drug coverage, so Humana does not offer a Medicare Advantage-only (MA-only, no drug) plan in the county next year.
Humana does not offer any Special Needs Plans in Los Angeles County for 2027 — no Dual-Eligible SNPs (D-SNP), no Chronic Condition SNPs (C-SNP), and no Institutional SNPs (I-SNP). Readers who qualify for a Special Needs Plan would need to look to another carrier.
The table below lists every Humana plan in the county for 2027 by name and type. We are not permitted to attach premiums, copays, deductibles, benefits, networks, provider lists or star ratings to any plan name, and nothing in this article should be read as a recommendation of one plan over another.
What's new, what's not returning
Compared with Humana's 2026 lineup in Los Angeles County, one plan is new for 2027: Humana Gold Plus H3767-002 (HMO). No Humana plans are leaving the county for 2027, and none have been renamed.
Even when a plan is "returning," the details inside it can shift year to year — copays, drug tiers, the pharmacy network, the provider directory, and extra benefits. Those changes are spelled out in the Annual Notice of Change (ANOC) that each plan mails to current members in September. If you are already in a Humana plan, the ANOC is the single most useful document to read before December 7.
HMO or PPO: what the plan type means
Humana's 2027 Los Angeles County lineup splits evenly: 4 HMO plans and 4 PPO plans. The plan type sets the ground rules for how you use the plan.
- HMO (Health Maintenance Organization): You generally use in-network doctors and hospitals, and you may need a referral to see a specialist.
- PPO (Preferred Provider Organization): You can usually go out of network, though your share of the cost is typically higher than when you stay in network.
Which structure fits depends on which doctors you want to keep seeing and how you prefer to get specialist care — not on which type sounds better on paper.
How to check a plan against your doctors and prescriptions
Before you choose or renew, check two things specific to you: your providers and your medications.
1. Go to medicare.gov/plan-compare, enter your ZIP code, and add your prescription drugs and preferred pharmacies. The Plan Finder will show each plan's drug costs for your specific list. 2. Confirm your doctors, hospitals and specialists are in-network by checking the plan's own online provider directory, or by calling the plan directly. Directories change throughout the year, so verifying close to enrollment is worthwhile. 3. Read the plan's Summary of Benefits and, if you are a current member, the Annual Notice of Change, to see what is different for 2027. 4. If you want to talk through options with Medicare directly, call 1-800-MEDICARE (1-800-633-4227), 24 hours a day, seven days a week. TTY users: 1-877-486-2048.
Dates and dollars to keep in mind
The Annual Enrollment Period runs October 15 through December 7, 2026. Any change you make in that window takes effect January 1, 2027. If you do nothing, your current plan simply renews for 2027 as long as it is offered — with whatever changes its ANOC describes. There is also a Medicare Advantage Open Enrollment Period from January 1 through March 31, 2027, which gives Medicare Advantage members one chance to switch to another Medicare Advantage plan or go back to Original Medicare.
A few program-level numbers to hold in mind as you compare drug coverage:
- Part D out-of-pocket cap: $2,100 in 2026, rising to $2,400 in 2027.
- Part D maximum deductible: $615 in 2026, rising to $700 in 2027.
- Part D national base beneficiary premium: $41.33 for 2027 (this is a formula input used by CMS, not what any particular plan charges).
- The coverage gap (the old "donut hole") was eliminated in 2025; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread your drug costs across the calendar year in monthly amounts, at no added cost.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes those figures later in the fall.
