FORT WAYNE, Ind. — The 2027 Medicare Advantage map in Allen County is shifting this fall. A new carrier, Devoted Health, is entering the county; ten plans will not come back after 2026; and sixteen plans are new for the coming year. Those changes take effect January 1, 2027, and the window to pick a plan for next year — the Annual Enrollment Period — runs October 15 through December 7, 2026.

Fort Wayne sits in Allen County, where Medicare Advantage availability is set. The county's 2026 lineup lists 58 plans from 7 carriers. The table below shows every plan by carrier and type. This article walks through what's new, what's leaving and what to do if your current plan is one of the ones not returning.

How many Medicare Advantage plans are offered in Allen County for 2027?

Allen County — the county that contains Fort Wayne — has 58 Medicare Advantage plans from 7 carriers in 2026, and the 2027 lineup reshuffles that total with 16 additions and 10 departures. Of the 2026 plans, 12 are HMOs, 12 are HMO-POS and 34 are PPOs. Twenty are Special Needs Plans (SNPs), and 50 include Part D drug coverage.

The 2026 roster, in alphabetical order by carrier, includes Aetna (CVS Health) with 4 PPO plans; American Health Advantage of Indiana with 1 HMO plan; Centene (WellCare) with 6 plans across HMO-POS and PPO; Elevance Health (Anthem) with 12 plans across HMO, HMO-POS and PPO; Humana with 19 plans across HMO, HMO-POS and PPO; Provider Partners Health Plans with 3 HMO plans; and UnitedHealthcare with 13 plans across HMO-POS and PPO. The table below lists every plan by name and type.

Which Medicare Advantage carrier is new to Fort Wayne for 2027?

Devoted Health is the new carrier entering Allen County for 2027. Its seven new offerings for the county are all PPOs: DEVOTED C-SNP CHOICE ENHANCED 015 IN, DEVOTED C-SNP CHOICE GIVEBACK EXTRAS 019 IN, DEVOTED C-SNP CHOICE PLUS 013 IN, DEVOTED CHOICE 001 IN, DEVOTED CHOICE GIVEBACK 002 IN, DEVOTED CHOICE GIVEBACK EXTRAS 024 IN and DEVOTED CHOICE PREMIUM 004 IN.

The other six carriers already selling in the county continue into 2027. Three of the Devoted entries are Chronic Condition Special Needs Plans (C-SNPs), which are designed for members with specific long-term conditions.

What Medicare Advantage plans are new for 2027 in Allen County?

Sixteen plans are new for 2027 in Allen County. In addition to the seven Devoted Health plans listed above, the new entries include Anthem Medicare Advantage 6 (HMO-POS) and Anthem Medicare Advantage 7 (HMO-POS) from Elevance Health; and from Humana, Humana Essentials Plus Giveback (PPO), Humana PathWays Dual Care NFLOC (HMO-POS D-SNP), Humana Senior Living Plan (PPO I-SNP), two Humana Together in Health (PPO I-SNP) offerings and HumanaChoice Giveback H5216-481 (PPO). Provider Partners Health Plans is adding Provider Partners Indiana Community Plan (HMO I-SNP).

Six existing plans have also been renamed, which is a paperwork change rather than a new plan: Anthem Medicare Advantage 3 and Anthem Medicare Advantage 4 are now HMO-POS instead of HMO; Humana Full Access (PPO) was Humana Full Access H5216-192 (PPO); and three UnitedHealthcare plans — UHC Dual Complete IN-Q1 (PPO D-SNP), UHC PathWays Dual Care IN-Y1 (PPO D-SNP) and UHC PathWays Dual Care IN-YL (PPO D-SNP) — have new plan IDs.

Which plans are not returning to Fort Wayne after 2026?

Ten plans will not be offered in Allen County for 2027. From Centene (WellCare): Wellcare Assist (HMO-POS), Wellcare Assist Open (PPO) and Wellcare Patriot Giveback Open (PPO). From Humana: Humana Gold Plus Chronic Kidney Disease (HMO C-SNP), Humana Together in Health (PPO I-SNP), two plans called Humana USAA Honor Giveback with Rx (PPO), HumanaChoice Giveback H5216-309 (PPO) and HumanaChoice Giveback H7617-003 (PPO). From Provider Partners Health Plans: Provider Partners Indiana Community Plan (HMO I-SNP) — note that a plan under the same name reappears as a new entry for 2027, which Medicare treats as a different contract.

If you are in one of these plans, you will get a non-renewal notice from the plan this fall. That notice triggers a Special Enrollment Period: you can switch to another Medicare Advantage plan or return to Original Medicare (with or without a stand-alone Part D plan) outside the usual window. The SEP for a non-renewed plan runs from December 8, 2026 through the end of February 2027. You can still make a choice during the regular Annual Enrollment Period as well, which usually gives the smoothest January 1 transition.

What should Fort Wayne residents do during the 2027 Annual Enrollment Period?

The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027. If you do nothing and your plan is continuing, it renews automatically with the updates described in its Annual Notice of Change (ANOC), which your plan mails in late September.

Twelve plans in the county are changing on at least one of premium, deductible, type or name for 2027 — the ANOC is where those specifics live. Forty-two plans continue as before. A few program-level numbers are already set for 2027: the Part D out-of-pocket cap rises to $2,400 (from $2,100 in 2026), and the Part D maximum deductible rises to $700 (from $615 in 2026). The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, are published by CMS later in the fall, usually in November.

The Medicare Prescription Payment Plan — the option to spread drug costs across the calendar year — is still available for 2027, and you can opt in through your Part D or Medicare Advantage plan.

Frequently asked questions

What happens if my Medicare Advantage plan is leaving Fort Wayne after 2026?
Your plan will mail you a non-renewal notice this fall. That notice gives you a Special Enrollment Period that runs from December 8, 2026 through the end of February 2027, so you can pick another Medicare Advantage plan or go back to Original Medicare with a stand-alone Part D plan. You can also make the change during the regular Annual Enrollment Period (October 15 – December 7, 2026), which lets your new coverage start on January 1, 2027 without a gap.
Do I have to do anything if my current plan is continuing into 2027?
No. If your plan is one of the 42 continuing in Allen County, it renews on its own. Read the Annual Notice of Change your plan mails in late September to see what is different for 2027 — including anything flagged on premium, deductible, plan type or name — and compare it to your other options on medicare.gov if you want to.
When will the 2027 Part B premium and IRMAA brackets be announced?
CMS publishes the 2027 Part B standard premium, Part B deductible and IRMAA income brackets later in the fall of 2026, typically in November. Until then, no one — including this site — has those final numbers. Watch medicare.gov or the official CMS announcement for the figures.
How do I compare the Fort Wayne plans without a sales call?
Use the Medicare Plan Finder at medicare.gov, or call 1-800-MEDICARE (TTY 1-877-486-2048), which is open 24 hours a day, seven days a week. You can enter your ZIP code and prescriptions and see every plan available in Allen County, including the new 2027 entries, side by side. If you want to look at the full 2027 picture for Fort Wayne, start with the plan table on this page, then open the Medicare Plan Finder at medicare.gov for the personalized details — premiums, networks and drug coverage — that this article does not include.
