What Aetna is offering in New York County for 2027
For plan year 2027, Aetna (which CMS lists as Aetna (CVS Health)) has seven Medicare Advantage plans available to people living in New York County. The mix by plan type is four HMOs and three PPOs, and every plan includes prescription drug coverage.
Four of the seven are Special Needs Plans — plans built around a specific group of enrollees:
- D-SNP (Dual Eligible): for people who have both Medicare and Medicaid.
- C-SNP (Chronic Condition): for people living with certain chronic illnesses.
- I-SNP (Institutional): for people who live in, or need the level of care provided in, a long-term care setting.
Aetna's 2027 lineup includes one D-SNP, two C-SNPs and one I-SNP. The table below lists every Aetna plan available in the county next year, by plan type and whether it includes drug coverage.
What's new, and what's not returning
Compared with 2026, there are three changes worth knowing about:
- Two new plans for 2027: Aetna Medicare Chronic Care Total (HMO C-SNP) and Aetna Medicare Elite (PPO).
- Three plans not returning: current enrollees in a plan that is leaving will get a non-renewal notice from the carrier explaining their options.
- Other plans in the lineup are continuing, but their benefits, drug formularies and provider networks can still change year to year. Those changes are spelled out in each plan's Annual Notice of Change (ANOC), which Aetna is required to send to current members by the end of September.
If a plan is not returning, you have a Special Enrollment Period to pick a new one — but it is simpler to make a choice during the regular Annual Enrollment Period that runs through December 7.
How to check a plan against your doctors and prescriptions
Before you choose or renew a plan, run three checks. Each takes a few minutes and can save real headaches in January.
1. Doctors and hospitals. Confirm every provider you want to keep is in the plan's 2027 network. Use the plan's provider directory on Aetna's own website for 2027, or call the plan directly. Networks can change on January 1. 2. Prescriptions. Enter each of your medications into the Medicare Plan Finder at medicare.gov or ask on 1-800-MEDICARE (TTY 1-877-486-2048). The tool shows which plans cover each drug, at what tier, and with what pharmacy restrictions. 3. Pharmacies. Make sure your usual pharmacy is a preferred or in-network pharmacy for the plan — this often affects what you pay at the counter.

The 2027 numbers CMS has confirmed
A few program-level figures are worth keeping in mind as you shop. These apply to Part D drug coverage across all Medicare plans — they are not specific to Aetna:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (up from $615 in 2026).
- The coverage gap, or "donut hole," has been eliminated since 2025; Part D now has three phases.
- The Medicare Prescription Payment Plan lets you spread drug costs across the year in monthly installments instead of paying at the pharmacy.
The 2027 Part B standard premium and deductible, and the IRMAA income brackets, had not been published as of September 2026. CMS usually releases those figures in the fall, often in November.
