Humana's 2027 Medicare Advantage plans in New York City: one leaves, one new, the full list

NEW YORK CITY — Fall 2026. If you have a Humana Medicare Advantage plan in New York City, or you're thinking about one for next year, this is the stretch of the calendar when it pays to look closely. Humana's 2027 lineup in New York County — the county New York City sits in for Medicare purposes — is smaller than it was this year, with one brand-new plan, one Humana plan not returning in the same form, and a few familiar names carrying over. The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027.

MyMedicarePlans.org is an independent consumer site. We are not Humana, and Humana is one of 15 carriers offering Medicare Advantage plans in New York County for 2027 (61 plans in all). This article walks through only Humana's slice of that lineup.

What Humana is offering in New York County for 2027

Humana has four Medicare Advantage plans in the county next year. By plan type, that's 2 HMO plans and 2 PPO plans. One of the HMOs is a D-SNP, built for people enrolled in both Medicare and Medicaid. All four plans include Part D prescription drug coverage (what the industry calls MA-PD).

The four plans, listed alphabetically exactly as CMS lists them:

The table on this page shows the full roster. We can't quote premiums, copays, deductibles, networks or star ratings here — those are plan-specific details that belong on medicare.gov and in each plan's own documents.

What's new, what's not returning

Compared with 2026, Humana's New York County lineup has:

If you're currently in one of the plans that is not returning, Humana is required to send you a non-renewal notice explaining your options. If you take no action, you'll generally go back to Original Medicare on January 1, 2027, and may want to pick a new Medicare Advantage or Part D plan during the Annual Enrollment Period or during the Special Enrollment Period that non-renewal triggers.

How to check a plan against your doctors and drugs before December 7

This is the most useful hour you'll spend this fall. Before you lock in a 2027 plan — Humana's or anyone else's — do these four things:

1. Pull out your ANOC. Your current plan mailed its Annual Notice of Change in September. It lists every 2027 change to your plan: cost-sharing, drug tier, formulary, network. 2. Make two short lists: your doctors and specialists (with the hospitals you prefer), and your prescriptions (name, dose, how often). 3. Run the plan through Medicare's Plan Finder at medicare.gov/plan-compare. Type in your ZIP code and your drug list, then check each plan's provider directory for your doctors and preferred hospital. 4. Call to confirm. Call the plan directly, or call 1-800-MEDICARE (1-800-633-4227), available 24/7. If a doctor matters to you, call that doctor's office and ask whether they will be in-network with the specific plan in 2027 — not just "with Humana."

For D-SNP eligibility (the Humana Gold Plus SNP-DE plan), you need to be enrolled in both Medicare and New York Medicaid. The plan will verify Medicaid status before enrollment.

The 2027 numbers worth knowing

A few program-wide figures CMS has already confirmed for next year:

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets for higher earners, have not yet been announced. CMS typically publishes them later in the fall.

If you do nothing

Nothing dramatic happens. If your 2026 plan is being offered again in 2027, you'll stay in it automatically with the changes described in the ANOC. If it isn't being offered — which is the case for three Humana plans in this county — you'll get a non-renewal letter and will need to pick something else or you'll return to Original Medicare.

You also have the Medicare Advantage Open Enrollment Period from January 1 to March 31, 2027, when anyone already in a Medicare Advantage plan can switch to a different MA plan or drop back to Original Medicare (with a Part D plan). That window is a safety valve, not a substitute for the fall review.
