What is new, renamed, or not returning
Four Humana plans are new to Jefferson County for 2027: Humana Dual QMB Only (HMO D-SNP), Humana Gold Plus H5619-188 (HMO), Humana Senior Living Plan (PPO I-SNP), and a new Humana Together in Health (PPO I-SNP) offering. Three plans are returning under new names: Humana Essentials Plus Giveback (PPO) was HumanaChoice Giveback H7617-049 (PPO); Humana Value Choice (PPO) was HumanaChoice H7617-050 (PPO); and HumanaChoice H5216-226 (PPO) was HumanaChoice Giveback H5216-226 (PPO).
Eight Humana plans sold in the county in 2026 are not being offered in 2027. If you are in one of them, Humana is required to mail you a non-renewal notice this fall explaining your options. You will not lose Medicare — but you will need to pick a new plan during AEP (through December 7) or you'll fall back to Original Medicare on January 1.
The Special Needs Plans, in plain English
Nine of the 21 Humana plans are Special Needs Plans. These are not open enrollment to everyone:
- D-SNPs (four Humana plans) are for people who have both Medicare and Medicaid.
- C-SNPs (one Humana plan, for diabetes and heart conditions) are for people with a qualifying chronic condition.
- I-SNPs (three Humana plans) are for people living in a long-term care facility or who need that level of care at home.
If you don't meet the eligibility rules, you can't enroll in an SNP, no matter how its benefits look.
How to check a plan before December 7
Whether you are staying with Humana, switching plans, or looking at another carrier, the checks are the same. Do them before December 7, 2026.
1. Read your ANOC. The Annual Notice of Change arrived (or will arrive) by late September. It compares your 2026 plan to its 2027 version — premium, deductible, copays, drug tiers, network changes. 2. Check your doctors. Go to medicare.gov's Plan Finder or call the plan directly and confirm every doctor, specialist and hospital you use is in network for 2027. Networks change every year. 3. Check your prescriptions. Enter each drug, dose and pharmacy into the Plan Finder. Formularies change too — a drug covered this year may be in a different tier, or require prior authorization, next year. 4. Confirm SNP eligibility if you're looking at a D-SNP, C-SNP or I-SNP. 5. Look at the total picture — premium plus expected drug costs plus expected medical copays — not any single number.
A couple of 2027 program facts worth knowing while you compare drug coverage: the Part D out-of-pocket cap rises to $2,400 in 2027 (up from $2,100 in 2026), and the Part D maximum deductible is $700 in 2027 (up from $615 in 2026). CMS has not yet published the 2027 Part B standard premium, deductible, or IRMAA income brackets; those usually come out in November.
If you do nothing
If your current Humana plan is being offered again in 2027, it renews automatically on January 1 — with whatever changes are in the ANOC. If your plan is one of the eight not returning, doing nothing means you'll be in Original Medicare starting January 1, 2027, without Part D drug coverage, unless you enroll in something else by December 7.
There's also a second window: the Medicare Advantage Open Enrollment Period, January 1 – March 31, 2027, which lets you make one switch from a Medicare Advantage plan to another Advantage plan or back to Original Medicare.
