Medicare Advantage
Does your Humana plan still cover your doctor in 2027?
Portland, Ore. — Fall 2026 is when Medicare's Annual Enrollment Period opens, and for Portland residents who have — or are considering — a Humana Medicare Advantage plan, this is the window to see what Humana is offering here next year and whether it still fits the doctors you see and the drugs you take. Humana is bringing eight Medicare Advantage plans to Multnomah County, the county Portland sits in, for the plan year that starts January 1, 2027. None are leaving, none are brand new, and three carry new names.

What Humana Medicare Advantage plans are offered in Portland for 2027?
In Multnomah County, the county Portland is in, Humana is offering 8 Medicare Advantage plans for 2027, according to the CMS Medicare Advantage Landscape Source File. All 8 include Part D drug coverage. By plan type, there are 4 HMOs and 4 PPOs. Two of them are Chronic Condition Special Needs Plans (C-SNPs) for people living with diabetes and heart conditions; Humana is not offering a Dual-Eligible SNP or an Institutional SNP in the county for 2027.
In alphabetical order, Humana's 2027 lineup here is:
- Humana Essentials Plus Giveback (HMO) — HMO, with drug coverage
- Humana Gold Plus – Diabetes and Heart (HMO C-SNP) — HMO C-SNP, with drug coverage
- Humana Gold Plus – Diabetes and Heart (HMO C-SNP) — HMO C-SNP, with drug coverage
- Humana Total Complete (HMO) — HMO, with drug coverage
- Humana USAA Honor Giveback (PPO) — PPO, with drug coverage
- Humana USAA Honor Giveback (PPO) — PPO, with drug coverage

The table below lists every Humana plan in the county with its plan type and drug-coverage status. For costs, benefits, provider networks and drug lists, use the Medicare Plan Finder at medicare.gov or the plan's Evidence of Coverage — we do not list those details here.
What is new or not returning from Humana in Portland for 2027?
Nothing is leaving and nothing is brand new. For 2027, Humana is not pulling any Medicare Advantage plans out of Multnomah County, and it is not introducing any new ones. Three of its plans, however, are being offered under new names:
- Humana Essentials Plus Giveback (HMO) was previously Humana Gold Plus Giveback H1036-323 (HMO).
- Humana Total Complete (HMO) was previously Humana Gold Plus H1036-153 (HMO).
- Humana Value Choice (PPO) was previously HumanaChoice H5216-428 (PPO).
A name change on its own does not tell you what shifted inside the plan. Premiums, copays, drug tiers and provider networks can change from one year to the next even when a plan keeps the same name — and especially when it does not. The Annual Notice of Change (ANOC) that Humana mails each member in September spells out what is different for 2027 versus 2026.
How do Portland residents check a Humana plan against their doctors and drugs before December 7?
Use the Medicare Plan Finder at medicare.gov, or call 1-800-MEDICARE (TTY 1-877-486-2048), before December 7, 2026. The Plan Finder lets you type in each prescription you take and each pharmacy you use, then shows which plans in Multnomah County cover those drugs and at what tier. You can also search by doctor to see which plans list a provider as in-network.
A short checklist before you decide:
- Read the ANOC for your current plan — Humana sends it in September. It lists every 2027 change.
- Make a list of your prescriptions, dosages and preferred pharmacy.
- Make a list of your doctors and any hospital or specialty clinic you want to keep using.
- On medicare.gov, compare plans side-by-side with your drug list loaded.
- Call each doctor's office to confirm they will still be in-network for the specific 2027 plan you are considering — directories can lag.
Formularies and networks are updated for the new plan year, so a plan that worked for you in 2026 may treat one of your drugs or doctors differently in 2027.
What does the 2027 Annual Enrollment Period mean for Portland Medicare members?
The Annual Enrollment Period runs October 15 through December 7, 2026, and any change you make takes effect January 1, 2027. If you do nothing, your current plan renews for 2027 as described in its ANOC — including any renamed version of it. After December 7, you still have the Medicare Advantage Open Enrollment Period from January 1 to March 31, 2027, during which someone already in a Medicare Advantage plan can switch to a different MA plan or go back to Original Medicare.
A few program-level numbers worth knowing as you compare drug coverage: for 2027, CMS has confirmed the Part D out-of-pocket cap will be $2,400 (up from $2,100 in 2026), the Part D maximum deductible will be $700 (up from $615 in 2026), and the national base beneficiary premium is set at $41.33 (that is a formula input, not what any specific plan charges). The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been published as of September 2026; CMS typically announces them in the fall.
