What is new, renamed or not returning
For 2027, UnitedHealthcare has one new plan in Philadelphia County: UHC Complete Care Support PA-1A (HMO-POS C-SNP), a Chronic Condition Special Needs Plan.
One plan has been renamed: what was UHC Dual Complete PA-V001 (HMO-POS D-SNP) in 2026 is now UHC Dual Advantage PA-V1 (HMO-POS D-SNP) for 2027. A new name does not automatically mean the benefits or network are the same — check the Annual Notice of Change (ANOC) your plan sent for the specifics.
No UnitedHealthcare plans in Philadelphia County are being discontinued for 2027.
Special Needs Plans in the lineup
Five of UnitedHealthcare's 14 Philadelphia-area plans are Special Needs Plans (SNPs), which have extra eligibility rules:
- D-SNP (Dual Eligible): for people who have both Medicare and Medicaid. UnitedHealthcare has 3 D-SNPs here — UHC Dual Advantage PA-V1, UHC Dual Complete PA-S001 and UHC Dual Complete PA-S002.
- C-SNP (Chronic Condition): for people with certain qualifying conditions. UnitedHealthcare has 2 C-SNPs — UHC Complete Care PA-17 and the new UHC Complete Care Support PA-1A.
- I-SNP (Institutional): for people who live in a nursing home or need that level of care. UnitedHealthcare has 1 I-SNP — UHC Nursing Home Plan EX-F002.
If you are not sure whether you qualify, call 1-800-MEDICARE or check at medicare.gov.
How to check a plan against your doctors and prescriptions
Before December 7, three quick checks can save a lot of second-guessing in January:
1. Pull up the Annual Notice of Change (ANOC) your current plan mailed in September. It lists what is changing for 2027 — premium, deductible, copays, drug tiers, extra benefits and network. 2. Run your drug list on the Medicare Plan Finder at medicare.gov. Type each prescription in with the dose, and the tool will show what each plan would cover and what it would cost at your pharmacy. 3. Call your doctors' offices and ask whether they will be in-network with the specific plan you are considering for 2027. Network status can change year to year, and the plan's own provider directory is the authoritative source.
What is confirmed for 2027 — and what is not
Some 2027 Medicare numbers are already set. The Part D out-of-pocket cap rises to $2,400 in 2027, up from $2,100 in 2026. The Part D maximum deductible is $700 in 2027, up from $615 in 2026. The Part D national base beneficiary premium for 2027 is $41.33; that is a formula input CMS uses, not what any particular plan charges.
The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not yet been announced. CMS typically publishes those in the fall, usually in November. Please do not rely on projections you see elsewhere — wait for the official number.
Also worth remembering: the coverage gap (the old "donut hole") has been gone since 2025, and Part D now has three phases. If drug costs land unevenly through the year, the optional Medicare Prescription Payment Plan lets you spread them across monthly bills.
If you do nothing
Nothing dramatic happens on December 8 if you take no action. Your current plan renews on January 1, 2027, with whatever changes the ANOC describes — unless your plan is being discontinued, in which case your carrier would have told you directly. None of UnitedHealthcare's Philadelphia plans fall into that category for 2027.
You also get a second chance: the Medicare Advantage Open Enrollment Period runs January 1 – March 31, 2027, and lets you switch Medicare Advantage plans or go back to Original Medicare once during that window.
