Fort Worth's 2027 Medicare Advantage map: who's in, who's out, who's arriving

Tarrant County has 83 Medicare Advantage plans for 2027. Compared with 2026, 28 plans are not coming back, 22 are new, and 32 continuing plans changed in some way you should check before December 7, 10 of them under a new name. This guide covers what those numbers mean for Fort Worth, not which plan to pick.

FORT WORTH, Texas — Fall 2026. If you have a Medicare Advantage plan in Fort Worth, this is the season to open the envelope. The 2027 lineup in Tarrant County is settled, and it looks different from this year's. CMS records show 28 plans will not be back next year, 22 are new, 10 others have been renamed, and one new carrier — Verda Health Plan of Texas — is joining the market. One carrier, American Health Advantage of Texas, will not offer plans here in 2027.

The Annual Enrollment Period runs October 15 through December 7, 2026. Any change you make during that window starts January 1, 2027. If you do nothing and your plan is still offered next year, it simply renews with the updates described in its Annual Notice of Change (ANOC).

What the 2027 Fort Worth lineup looks like

Fort Worth is in Tarrant County, where Medicare Advantage availability is set at the county level by CMS. For 2026, the county had 89 Medicare Advantage plans from 12 carriers — a mix of HMO, HMO-POS, PPO and PFFS plans, with 36 Special Needs Plans (SNPs) included. Every plan currently offered here includes drug coverage.

Plans not returning after 2026

Twenty-eight plans in Tarrant County will not be offered in 2027. If your plan is on this list, your insurer will send a non-renewal notice, and you'll want to pick a new plan during the fall window. Plans leaving, by carrier:

If your plan is not being renewed, you qualify for a Special Enrollment Period. It generally runs from December 8, 2026 through the end of February 2027, giving you extra time to switch to another Medicare Advantage plan or return to Original Medicare and add a stand-alone Part D plan.

Renamed and otherwise changed plans

Ten plans continuing into 2027 have a new name. If your card next year reads differently from this year's, you may be looking at one of these:

Separately, 22 continuing plans in Tarrant County have at least one change in premium, deductible, plan type, or name for 2027. Your Annual Notice of Change spells out what's different for your specific plan.

Program-level numbers to keep in mind for 2027

A few 2027 figures CMS has already confirmed apply no matter which plan you pick:

• Part D out-of-pocket cap: $2,400 in 2027 (up from $2,100 in 2026).
• Part D maximum deductible: $700 in 2027 (up from $615 in 2026).
• Part D national base beneficiary premium: $41.33 for 2027. This is a formula input used across the program, not what any single plan charges.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, have not been announced yet. CMS typically publishes them later in the fall — usually in November.

What to do during the window

• Read your plan's Annual Notice of Change when it arrives. It lists every change for 2027.
• If your plan is leaving, watch for the non-renewal letter and use your Special Enrollment Period if you need more time.
• Use the Medicare Plan Finder at medicare.gov to compare 2027 plans side by side, with your prescriptions and doctors entered.
