SAN ANTONIO — As the Annual Enrollment Period opens on October 15, 2026, the Medicare Advantage menu in Bexar County is set to look noticeably different in 2027. One carrier is leaving the county, a new carrier is joining, and more than two dozen plans are being added while a similar number step away. If you live in or near San Antonio and have a Medicare Advantage plan, this is the season to read your Annual Notice of Change and see where your plan lands.

What's changing in San Antonio for 2027

San Antonio sits in Bexar County, and that is the level at which Medicare sets plan availability. For 2026, Bexar County residents can choose from 100 Medicare Advantage plans offered by 15 carriers. The mix includes HMO, HMO-POS, PPO and PFFS plans, and 41 of them are Special Needs Plans for people with certain chronic conditions, Medicaid eligibility, or who live in an institutional setting.

The carrier picture: one out, one in

At the carrier level, the biggest shift is at the edges of the market.

American Health Advantage of Texas is not offering a Medicare Advantage plan in Bexar County for 2027. Its single plan here, American Health Advantage of Texas (HMO I-SNP), is leaving.

Alignment Health Plan is new to the county for 2027. It is entering with six HMO-POS plans: Alignment Health BRIDGE 013, Alignment Health Duals+ 007, Alignment Health Heart & Diabetes 002, Alignment Health Heart & Diabetes Plus 004, Alignment Health ONE 011, and Alignment Health Smart 012.

The other 14 carriers from 2026 are all still here for 2027, though several are reshuffling their lineups. Humana remains the largest carrier in the county by plan count, followed by UnitedHealthcare and Blue Cross Blue Shield (HCSC).

Plans that are not returning

If your plan appears on the list below, it will not be offered in Bexar County in 2027. Your carrier will send you a non-renewal notice, and you qualify for a Special Enrollment Period to choose a different Medicare Advantage plan or return to Original Medicare (with the option to add a stand-alone Part D plan).

A non-renewed plan gets you a Special Enrollment Period that extends past December 7, giving you time to pick a replacement without a gap in coverage.

Plans new for 2027

Alongside Alignment Health's six HMO-POS plans, the following plans are new in Bexar County for 2027:

Many of the new plans are Special Needs Plans, which are limited to people who qualify by chronic condition, dual Medicare-Medicaid eligibility, or institutional status.

Renamed plans — same plan, new label

Eleven plans in the county are keeping their footprint but changing names. If one of these is yours, you don't need to re-enroll; the plan simply has a new name on paperwork. They include Aetna Medicare Chronic Care (HMO C-SNP), Aetna Medicare Full Dual Care (HMO D-SNP), Aetna Medicare Full Dual Extra Care (HMO D-SNP), Aetna Medicare QMB Only Extra Care (HMO D-SNP), Aetna Medicare Signature Extra (PPO), Wellcare Superior HealthPlan Dual Access (HMO D-SNP), Wellcare Superior HealthPlan Dual Reserve (HMO D-SNP), Community First Medicare Advantage Bexar Dual (HMO D-SNP), DEVOTED PREMIUM 004 TX (HMO), Wellpoint Medicare Advantage Select (HMO-POS) and AARP Medicare Advantage from UHC TX-61 (Regional PPO).

Even when the name is the only change, read the Annual Notice of Change for next year's premium, deductible, copays and provider network.

How to decide this fall

A few steps that work for most San Antonio shoppers:

1. Read your Annual Notice of Change. Carriers mailed these in September. It tells you what your 2027 plan looks like and whether it is being discontinued. 2. Make a short list of must-haves. Your doctors, your hospital system, the specific prescriptions you take, and whether you want an HMO (lower cost, in-network only) or a PPO (more flexibility outside the network). 3. Use the Medicare Plan Finder at medicare.gov to see premiums, deductibles, drug costs and star ratings side by side. 4. Check Part D math for 2027. The out-of-pocket cap on covered prescriptions rises to $2,400 (up from $2,100 in 2026), and the maximum Part D deductible a plan can charge is $700 (up from $615 in 2026). The Medicare Prescription Payment Plan lets you spread drug costs across the year in level monthly payments. 5. Watch the date. Enrollment choices made between October 15 and December 7, 2026 take effect January 1, 2027. If you do nothing and your plan is continuing, you'll be renewed into its 2027 version automatically.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, are set by CMS and typically announced in November — once published, you can plug them into your full-year budget.

Your carrier will send a non-renewal notice this fall. You qualify for a Special Enrollment Period that runs past the usual December 7 deadline, giving you time to pick another Medicare Advantage plan or return to Original Medicare and add a stand-alone Part D drug plan. If you do nothing, you'll generally be moved to Original Medicare on January 1, 2027, without drug coverage.
Do I have to re-enroll if my plan was just renamed?
No. A renamed plan is the same contract with a new label. You stay enrolled automatically. Still read the Annual Notice of Change, because premiums, drug tiers or networks may have shifted even if the name change is the headline.
What is the difference between an HMO, HMO-POS, PPO and PFFS plan?
