Wellcare's 2027 lineup in Tarrant County

Fort Worth is in Tarrant County, where Medicare Advantage availability is set at the county level. For 2027, CMS lists 6 Wellcare plans here, all HMOs, all including Part D drug coverage. Four of those are D-SNPs — Dual Special Needs Plans — which are for people who have both Medicare and Medicaid.

The table below lists every Wellcare plan offered in the county for 2027. For context, 11 other carriers also offer Medicare Advantage plans in Tarrant County next year, for 83 Medicare Advantage plans in total.

What is new, what is renamed, what is not returning

Two Wellcare plans are new in Tarrant County for 2027: Wellcare Superior HealthPlan Dual Liberty (HMO D-SNP) and Wellcare Superior HealthPlan Dual Reserve Value (HMO D-SNP). Both are D-SNPs with drug coverage.

Two plans come back with new names. Wellcare Superior HealthPlan Dual Access (HMO D-SNP) is the renamed version of Wellcare Dual Access (HMO D-SNP), and Wellcare Superior HealthPlan Dual Reserve (HMO D-SNP) is the renamed version of Wellcare Dual Reserve (HMO D-SNP). A rename can involve more than a new label — look at the ANOC for specifics on your benefits, network and drug list.

Two Wellcare plans in the current year's lineup are not returning for 2027. If you are in a plan that is leaving the county, Medicare gives you a Special Enrollment Period, but it is simpler to pick a 2027 plan during the Annual Enrollment Period that is already running.

Special Needs Plans explained

A Dual Special Needs Plan is for people enrolled in both Medicare and Medicaid. Four of Wellcare's six Tarrant County plans for 2027 are D-SNPs. They are not open to everyone — eligibility rules apply, and Medicare Plan Finder will show you which plans you qualify for when you enter your information.

Wellcare does not offer Chronic Condition SNPs (C-SNPs) or Institutional SNPs (I-SNPs) in the county for 2027.

Checking a plan against your doctors and prescriptions

Before you choose or stay in a plan, two checks matter most:

1. Your doctors and hospitals. Each Medicare Advantage HMO has its own network. Call your providers directly and ask whether they will be in-network for the specific plan you are considering in 2027 — networks can change at the start of the year.

2. Your prescriptions. Every plan with drug coverage has its own formulary (drug list) and its own pharmacy network. On medicare.gov, the Plan Finder lets you enter each of your medications and your preferred pharmacy and see the plan's cost estimate for the year.

A few program-level numbers to keep in mind as you compare drug coverage: for 2027, the Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible rises to $700 (from $615 in 2026). The coverage gap (the "donut hole") was eliminated in 2025, so Part D now has three phases. CMS has not yet announced the 2027 Part B standard premium, deductible, or the 2027 IRMAA income brackets — those usually come out in the fall.

Key dates and what happens if you do nothing

The Annual Enrollment Period runs October 15 – December 7, 2026. A change you make in that window takes effect January 1, 2027. If you do nothing and your current plan is offered again next year, it renews automatically with any changes described in its ANOC.

After the Annual Enrollment Period, people already enrolled in a Medicare Advantage plan have the Medicare Advantage Open Enrollment Period from January 1 to March 31, 2027, to make a one-time switch to another Medicare Advantage plan or back to Original Medicare.

Frequently asked questions

How do I find out if my doctor takes a specific Wellcare plan in Fort Worth for 2027?
Call your doctor's office directly and ask whether they will be in-network for the specific plan next year. You can also check the plan's provider directory at wellcare.com or ask when you call Medicare at 1-800-MEDICARE. Networks can change on January 1, so confirm for 2027, not just today.
My Wellcare plan was renamed. Do I need to do anything?
If your plan is renamed and continuing, your coverage will roll over automatically. Still, read the Annual Notice of Change your plan mailed you — a new name can come with changes to premiums, copays, the drug list or the network. If those changes do not work for you, you can switch to a different plan by December 7, 2026.
What happens if my Wellcare plan is not returning for 2027?
You will get a non-renewal notice from the plan. You can pick a new Medicare Advantage or Part D plan during the Annual Enrollment Period (through December 7, 2026), and Medicare also gives you a Special Enrollment Period that extends into early 2027 if your plan is leaving. If you make no change, you will generally return to Original Medicare without drug coverage.
Is a D-SNP right for me?
D-SNPs are only for people who have both Medicare and Medicaid. If you qualify, they coordinate the two programs and typically include drug coverage. If you are not sure whether you qualify, use medicare.gov's Plan Finder, which only shows you plans you are eligible for, or call 1-800-MEDICARE. ## Where to go next For personalized answers — including which plans your doctors and prescriptions line up with — use the Plan Finder at medicare.gov or call 1-800-MEDICARE (TTY 1-877-486-2048), available 24 hours a day, seven days a week. For plan-specific details, your plan's Evidence of Coverage, Annual Notice of Change, and provider and pharmacy directories are the official sources.
