What Medicare Advantage plans does Anthem offer in San Antonio for 2027?

Anthem — which CMS lists under its parent, Elevance Health — is offering 9 Medicare Advantage plans in Bexar County, the county that contains San Antonio, for the 2027 plan year. All 9 include Part D prescription drug coverage.

By plan type, the mix is 4 HMO plans and 5 HMO-POS plans. Seven of the nine are Special Needs Plans: 3 Dual-Eligible SNPs (for people who have both Medicare and Medicaid) and 4 Chronic Condition SNPs (for people with qualifying conditions such as certain chronic kidney or cardiovascular diseases). Anthem is not offering an Institutional SNP in the county for 2027.

Anthem's plans in San Antonio for 2027 are sold under the Wellpoint brand. In alphabetical order, they are: Wellpoint Chronic Care (HMO-POS C-SNP), Wellpoint Chronic Care Advantage Select (HMO C-SNP), Wellpoint Chronic Care Select (HMO-POS C-SNP), Wellpoint Dual Advantage Plus Select (HMO D-SNP), Wellpoint Dual Advantage Select (HMO D-SNP), Wellpoint Full Dual Advantage Select (HMO D-SNP), Wellpoint Kidney Care (HMO-POS C-SNP), Wellpoint Medicare Advantage 2 (HMO-POS), and Wellpoint Medicare Advantage Select (HMO-POS). The table below lists every plan with its type and drug coverage.

What is new or not returning from Anthem in Bexar County for 2027?

Five Anthem plans are new for 2027 in Bexar County, three 2026 plans are not returning, and one keeps its contract but changes its name.

The 5 new Anthem plans for 2027 are Wellpoint Chronic Care Advantage Select (HMO C-SNP), Wellpoint Chronic Care Select (HMO-POS C-SNP), Wellpoint Dual Advantage Plus Select (HMO D-SNP), Wellpoint Dual Advantage Select (HMO D-SNP), and Wellpoint Full Dual Advantage Select (HMO D-SNP).

One plan is renamed: Wellpoint Select (HMO-POS) is now called Wellpoint Medicare Advantage Select (HMO-POS). It is the same contract, with a new name. Three Anthem plans offered in 2026 are not being offered in 2027. If you are enrolled in one of those, Anthem will send you a notice explaining your options; your coverage will not simply disappear mid-year, but you will need to pick a new plan during Annual Enrollment (or you may be enrolled in a comparable plan — your notice will say).

For the exact changes to any plan that is continuing — things like the drug formulary, the provider network, copays or extra benefits — read the Annual Notice of Change (ANOC) your plan mails each fall. Those documents are the authoritative source, and we are not permitted to summarize plan-specific costs or benefits here.

How do I check an Anthem plan against my doctors and prescriptions?

Before December 7, 2026, run every plan you are considering through three checks: your doctors, your drugs and your pharmacy.

1. Doctors and hospitals. Use the Medicare Plan Finder at medicare.gov or call the plan directly to confirm each provider you see — primary care, specialists and any hospital — is in-network for the 2027 plan year. Networks can change year to year, even if the plan name stays the same. 2. Prescriptions. On medicare.gov, enter each drug, dose and quantity. The Plan Finder will show which plans cover each drug on their 2027 formulary and estimate your yearly drug cost under each one. 3. Pharmacy. Confirm your preferred pharmacy is a preferred or standard in-network pharmacy for 2027. The same drug can cost different amounts at different pharmacies in the same plan.

If anything is unclear, call 1-800-MEDICARE (TTY 1-877-486-2048), available 24/7, or call the plan using the number on its materials. You can also read the plan's Evidence of Coverage and provider and pharmacy directories on the plan's own website.

What else is changing in Medicare for 2027?

A few program-wide numbers are already set for 2027 and apply no matter which plan you pick.

The 2027 Part B standard premium and deductible, and the 2027 IRMAA income brackets, had not been announced as of this writing. CMS typically publishes them later in the fall. We will update our coverage when the official figures are released.

What happens if I do nothing before December 7?

If you take no action, your current plan rolls over into 2027 — provided the plan is still offered — with the changes described in your ANOC. Nothing is cancelled for you, and you do not need to re-enroll to keep what you have.

But if your plan is one of the three Anthem plans not returning in 2027, or if your doctor, drug or pharmacy situation has changed, Annual Enrollment (October 15 – December 7, 2026) is the window to make a change for a January 1, 2027 start date. There is also a Medicare Advantage Open Enrollment Period from January 1 – March 31, 2027, when people already in a Medicare Advantage plan can switch once or return to Original Medicare.

Remember that Anthem is one of 15 carriers offering Medicare Advantage in Bexar County for 2027, and 98 Medicare Advantage plans are offered in the county across all carriers. You have options to compare.

Frequently asked questions

When can I change my Medicare plan for 2027 coverage?
The Annual Enrollment Period runs October 15 through December 7, 2026. Changes you make during that window take effect January 1, 2027. If you are already in a Medicare Advantage plan on January 1, you also get the Medicare Advantage Open Enrollment Period from January 1 through March 31, 2027, when you can switch Medicare Advantage plans once or return to Original Medicare.
What is a Special Needs Plan, and do I qualify for one of Anthem's in San Antonio?
A Special Needs Plan (SNP) is a Medicare Advantage plan built for a specific group. A D-SNP is for people enrolled in both Medicare and Medicaid; a C-SNP is for people with certain chronic conditions, such as qualifying kidney disease. Anthem is offering 3 D-SNPs and 4 C-SNPs in Bexar County for 2027. Whether you qualify depends on your Medicaid status or diagnosis — the plan verifies eligibility before enrolling you. Medicare.gov and 1-800-MEDICARE can walk you through the specifics.
My Anthem plan is not being offered in 2027. What should I do?
Watch for a non-renewal letter from Anthem; it explains your options and the special enrollment window attached to a plan non-renewal. You can use the Annual Enrollment Period (through December 7, 2026) to pick a different Medicare Advantage plan, or you can return to Original Medicare and, if you want drug coverage, add a stand-alone Part D plan. Running your doctors and prescriptions through the Plan Finder at medicare.gov is the fastest way to compare.
Does Anthem offer the only Medicare Advantage plans in San Antonio?
No. Anthem is one of 15 carriers offering Medicare Advantage plans in Bexar County for 2027, and 98 Medicare Advantage plans are offered in the county across all carriers. This article focuses on Anthem's lineup because that is its subject; it is not a recommendation of Anthem over any other carrier. --- For personalized help comparing plans, visit **medicare.gov** or call **1-800-MEDICARE (1-800-633-4227)**, available 24 hours a day, 7 days a week.
