What Humana Medicare Advantage plans are offered in St. Louis for 2027?
Humana is offering 17 Medicare Advantage plans in St. Louis city for 2027. By type, that is 5 HMO plans, 2 HMO-POS plans, 9 PPO plans and 1 PFFS plan. Fourteen of the 17 include Part D drug coverage; the other three do not.
The table below lists every Humana plan available in the county for 2027, with its plan type and whether it includes drug coverage. Plan availability data comes from the CMS Medicare Advantage Landscape Source File for plan year 2027.
A quick note on plan types, since Humana offers four of them here:
- HMO plans generally ask you to use in-network providers and get referrals.
- HMO-POS adds a limited point-of-service option to go out of network for certain services.
- PPO plans let you see out-of-network providers, usually at higher cost.
- PFFS (Private Fee-for-Service) plans set the terms of payment; providers decide whether to accept them.
Which Humana plans in St. Louis are Special Needs Plans?
Three of Humana's 17 plans in St. Louis city for 2027 are Special Needs Plans (SNPs), which are built for people in specific situations. Two are Dual Eligible SNPs (D-SNPs) for people who have both Medicare and Medicaid: Humana Gold Plus SNP-DE H0028-015 (HMO-POS D-SNP) and Humana Gold Plus SNP-DE H4461-044 (HMO-POS D-SNP). One is a Chronic Condition SNP (C-SNP) for people with diabetes and heart conditions: Humana Gold Plus - Diabetes and Heart (HMO C-SNP).
You can only enroll in a SNP if you meet its eligibility rules. Medicare.gov or 1-800-MEDICARE can confirm whether you qualify.
What's new or not returning from Humana in St. Louis for 2027?
Nothing is leaving and nothing is brand new. Zero Humana plans in St. Louis city are not returning for 2027, and zero are new. What has changed is naming: 8 of Humana's 17 plans here have been renamed for 2027.
The renames include:
- Humana Essentials Plus Giveback (HMO) — previously Humana Gold Plus Giveback H0028-065 (HMO)
- Humana Essentials Plus Giveback (HMO) — previously Humana Essentials Plus Giveback H4461-045 (HMO)
- Humana Full Access (PPO) — previously Humana Full Access H5216-407 (PPO)
- Humana Full Access (PPO) — previously Humana Full Access H7617-008 (PPO)
- Humana Total Complete (HMO) — previously Humana Gold Plus H0028-014 (HMO)
- Humana Total Complete (HMO) — previously Humana Total Complete H4461-046 (HMO)
- Humana Value Choice (PPO) — previously Humana Value Choice H5216-318 (PPO)
- Humana Value Choice (PPO) — previously Humana Value Choice H7617-023 (PPO)
A rename doesn't automatically mean the plan is different inside — but it doesn't mean it's the same, either. The Annual Notice of Change (ANOC) that Humana mails each fall is where the specifics live: changes to the premium, deductible, copays, the provider network, the drug formulary, and extra benefits. If you got an ANOC in September, that's the document to pull out.
How do I check a Humana plan against my doctors and prescriptions before December 7?
The most reliable way is the Medicare Plan Finder at medicare.gov. It lets you type in your prescriptions and preferred pharmacies and see how each plan handles them, and it links out to each plan's provider directory so you can check your doctors and hospitals. You can also call 1-800-MEDICARE (TTY 1-877-486-2048), 24 hours a day, 7 days a week.
A simple checklist for each plan you're seriously considering:
1. Doctors and hospitals. Confirm each one is in-network for 2027 — networks reset at the plan level each year. Call the provider's office directly if you're unsure; they'll know which plans they accept. 2. Prescriptions. Enter every drug, dose and quantity into Plan Finder. The formulary (the plan's drug list), the tier each drug sits on and the preferred pharmacies can all shift year to year. 3. Pharmacy. Check whether your pharmacy is a preferred pharmacy for the plan, which often changes what you pay. 4. Your ANOC. If you're already in a Humana plan, read what changed for 2027 before deciding whether to stay.
For drug coverage in 2027, two program-level numbers are already set by CMS: the Part D out-of-pocket cap rises to $2,400 (up from $2,100 in 2026), and the Part D maximum deductible is $700 (up from $615 in 2026). Those are ceilings that apply across Part D plans; the actual deductible and cost-sharing in any specific Humana plan are listed in that plan's materials.
What happens if I do nothing by December 7, 2026?
Nothing dramatic. If your current Humana plan is offered again in St. Louis for 2027 — and all 17 of Humana's 2026 plans are returning in some form — you stay enrolled on January 1, 2027, with whatever changes the ANOC describes. You do not need to re-enroll each year.
If you decide after January 1 that your Medicare Advantage plan isn't working, the Medicare Advantage Open Enrollment Period runs January 1 through March 31, 2027. During that window you can switch to a different Medicare Advantage plan once, or go back to Original Medicare (and add a stand-alone Part D plan).
Two 2027 figures are not yet published: the standard Part B premium and deductible, and the IRMAA income brackets. CMS usually announces those in the fall. We'll update when they're out.
