North Las Vegas, Nev. — Fall 2026. If you have both Medicare and Medicaid, the plan choices you see this Annual Enrollment Period are different from what your neighbors see. North Las Vegas is in Clark County, where 16 Dual Special Needs Plans (D-SNPs) from six carriers will be offered for 2027 — a slate built specifically for people who qualify for both programs. This guide walks through who those plans are for, when a dual-eligible person can change coverage, and how to confirm your Medicaid status before you decide.
What is a Dual Special Needs Plan, and who is it for in North Las Vegas?
A Dual Special Needs Plan (D-SNP) is a type of Medicare Advantage plan built for people who have both Medicare and Medicaid. In North Las Vegas, that means you must be enrolled in Medicare (Parts A and B) and also qualify for Nevada Medicaid in some form.
Dual eligibility comes in two broad flavors:
- Full dual eligible. You qualify for full Medicaid benefits in addition to Medicare. Medicaid helps with your Medicare premiums and cost-sharing and also covers services Medicare does not, such as long-term services and supports.
- Partial dual eligible. You qualify for one of the Medicare Savings Programs (such as QMB, SLMB or QI) that help pay Medicare premiums or cost-sharing, but you do not get full Medicaid benefits.
Some D-SNPs are designed for the full-benefit group, others for the partial group, and some accept both. The plan's name and materials spell this out — for example, Clark County's 2027 lineup includes an "Aetna Medicare Full Dual" plan and an "Aetna Medicare Partial Dual" plan.
How many D-SNPs are available in Clark County for 2027?
For plan year 2027, Clark County will have 16 D-SNPs out of 72 Medicare Advantage plans overall. Six carriers offer them: Aetna (CVS Health) with 2, Alignment Health Plan with 1, Centene (WellCare) with 2, Elevance Health (Anthem) with 2, Humana with 3, and UnitedHealthcare with 6. Clark County will also have 16 Chronic Condition SNPs (C-SNPs) and 1 Institutional SNP (I-SNP) for people who meet those separate criteria.
A few plans in the D-SNP lineup are new for 2027 and a few have been renamed. If you were in a D-SNP in 2026, read the Annual Notice of Change (ANOC) your plan mailed you — it describes exactly what is changing, including a new name if your plan was renamed.
The full table below lists every 2027 D-SNP in the county by carrier, in alphabetical order. In plain prose, the carriers and plan names are:
- Aetna (CVS Health): Aetna Medicare Full Dual (HMO D-SNP) — new for 2027; Aetna Medicare Partial Dual (HMO D-SNP) — renamed.
- Alignment Health Plan: Alignment Health Duals+ 005 (HMO D-SNP) — renamed.
- Centene (WellCare): Wellcare SilverSummit Healthplan Dual Access (HMO-POS D-SNP) — new for 2027; Wellcare SilverSummit Healthplan Dual Liberty (HMO-POS D-SNP) — renamed.
- Elevance Health (Anthem): Anthem Full Dual Advantage (HMO D-SNP); Anthem Full Dual Advantage 2 (HMO D-SNP).
- Humana: Humana Gold Plus SNP-DE H6622-079 (HMO D-SNP); Humana Gold Plus SNP-DE H6622-101 (HMO D-SNP); HumanaChoice SNP-DE H5216-302 (PPO D-SNP).
- UnitedHealthcare: UHC Dual Complete NV-Q1 (HMO-POS D-SNP) — renamed; UHC Dual Complete NV-Q2 (PPO D-SNP) — renamed; UHC Dual Complete NV-Q3 (PPO D-SNP) — new for 2027; UHC Dual Complete NV-S3 (PPO D-SNP); UHC Dual Complete NV-S5 (HMO-POS D-SNP); UHC Dual Complete NV-S6 (PPO D-SNP) — new for 2027.
For premiums, drug lists, provider networks and specific benefits, use the Medicare Plan Finder at medicare.gov or request the Evidence of Coverage from the plan.
How do I confirm I'm eligible for a D-SNP in Nevada?
Medicare eligibility is confirmed by the Social Security Administration (ssa.gov or your Social Security account). Medicaid eligibility — including whether you are full or partial dual eligible — is confirmed by the Nevada Division of Welfare and Supportive Services, the agency that runs Medicaid in the state. You can also look at the back of your Medicaid card or your most recent award letter.
If you are not sure which category you fall into, do this before you enroll:
1. Call Nevada Medicaid or sign in to your Access Nevada account to see your current eligibility status. 2. Call 1-800-MEDICARE (TTY 1-877-486-2048), available 24/7, to confirm what Medicare has on file and which Special Enrollment Periods you qualify for. 3. Visit medicare.gov/plan-compare to see the D-SNPs available at your North Las Vegas ZIP code.
A plan can only enroll you in a D-SNP if your eligibility is verified, so getting this confirmed up front saves time.
How often can a dual-eligible person change plans?
Most people with Medicare can only change Medicare Advantage or Part D plans during the Annual Enrollment Period, Oct. 15 – Dec. 7, 2026, with coverage starting Jan. 1, 2027. Dual-eligible people have more flexibility.
Under rules that took effect in 2025 and remain in place:
- People with Medicaid or Extra Help generally have a monthly Special Enrollment Period to make one change — either to enroll in an integrated D-SNP (a D-SNP closely aligned with their Medicaid plan) or to drop Medicare Advantage and return to Original Medicare with a stand-alone Part D plan.
- Losing or gaining Medicaid, or moving between full and partial dual status, also opens a Special Enrollment Period.
- The Medicare Advantage Open Enrollment Period (Jan. 1 – March 31) lets anyone in a Medicare Advantage plan switch to another Medicare Advantage plan or go back to Original Medicare once.
These rules change from year to year. Before you act, confirm the current Special Enrollment Period rules with 1-800-MEDICARE.
What's changing for 2027 that D-SNP members in North Las Vegas should know?
Two things stand out this fall:
- The D-SNP lineup itself is shifting. Four of the 16 D-SNPs are new for 2027 and four were renamed. If your 2026 plan is renamed, you don't need to re-enroll, but the plan may have other changes — read your ANOC.
- Part D cost protections rise again. For 2027, the Part D out-of-pocket cap is $2,400 (up from $2,100 in 2026) and the Part D maximum deductible is $700 (up from $615 in 2026). D-SNP members whose drug costs are covered by Medicaid or Extra Help may not feel these caps directly, but they are the backdrop to how every Part D plan is built. CMS has not yet announced the 2027 Part B premium, Part B deductible or IRMAA income brackets — those typically come in November.
If you do nothing during AEP and your current plan is offered next year, you'll stay in it automatically for 2027, with whatever changes the ANOC describes.
