Anthem's 2027 Medicare Advantage plans in Indianapolis: two new, two renamed
INDIANAPOLIS — As the Annual Enrollment Period for 2027 Medicare coverage runs October 15 through December 7, 2026, Anthem's Medicare Advantage lineup in Marion County — the county Indianapolis sits in — is shifting at the edges. According to the CMS Medicare Advantage Landscape Source File for the 2027 plan year, Elevance Health (Anthem) will offer 16 Medicare Advantage plans in the county, up from 14 this year. Two plans are new, two have been renamed, and none of its current plans are leaving.

Key takeaways
- Anthem will offer 16 Medicare Advantage plans in Marion County for 2027 — 6 HMOs, 5 HMO-POS plans and 5 PPOs — and all 16 include drug coverage.
- Four of those are Dual Eligible Special Needs Plans (D-SNPs) for people who have both Medicare and Medicaid; Anthem is not offering a C-SNP or I-SNP here.
- Two plans are new for 2027 and two have been renamed; no Anthem plan in the county is going away.
- The deadline to make a change for January 1, 2027 coverage is December 7, 2026 — if you do nothing, your current plan simply renews with whatever changes are in its Annual Notice of Change.
This article is a plan-by-plan look at what Anthem is offering here next year. MyMedicarePlans.org is an independent consumer site and is not Anthem; 10 other carriers also offer Medicare Advantage plans in Marion County for 2027, for a county-wide total of 79 plans.
What Anthem is offering in Marion County for 2027
The table below lists every Anthem plan available in the county for 2027, with its plan type and whether it includes drug coverage. In short:
- 6 HMO plans
- 5 HMO-POS plans
- 5 PPO plans (including a regional PPO)
- 4 Dual Eligible Special Needs Plans (D-SNPs)
- All 16 plans include Part D drug coverage
Because Medicare Advantage availability is set at the county level, these are the Anthem plans a person with a Marion County ZIP code can shop. If you live in a neighboring county, your lineup may look different even if your doctor is in Indianapolis.
What is new, renamed or not returning
Compared with Anthem's 2026 lineup in the county, two plans are brand new for 2027:
- Anthem Medicare Advantage 6 (HMO-POS)
- Anthem Medicare Advantage 7 (HMO-POS)
Two plans kept their core identity but changed names and plan type on the CMS filing:
- Anthem Medicare Advantage 3 (HMO-POS) — previously Anthem Medicare Advantage 3 (HMO)
- Anthem Medicare Advantage 4 (HMO-POS) — previously Anthem Medicare Advantage 4 (HMO)
No Anthem plan offered in Marion County this year is being discontinued for 2027. If you are already enrolled in an Anthem plan here, it will renew on January 1, 2027 unless you choose a different plan during the Annual Enrollment Period. The specifics — any changes to the premium, the drug list, the provider network, cost-sharing or extra benefits — live in your Annual Notice of Change (ANOC), which plans mail out in late September. Read it before you decide whether to stay or switch.
The Special Needs Plans in the lineup
Anthem is offering four D-SNPs in the county for 2027. These plans are limited to people who qualify for both Medicare and Indiana Medicaid, and they coordinate benefits across the two programs:
- Anthem Dual Advantage (HMO D-SNP)
- Anthem Full Dual Advantage (HMO D-SNP)
- Anthem I PathWays Dual Care Advantage (HMO D-SNP)
- Anthem I PathWays Dual Care Advantage NFLOC (HMO D-SNP)
Anthem is not offering a Chronic Condition SNP (C-SNP) or an Institutional SNP (I-SNP) in Marion County for 2027. If you do not have Medicaid, a D-SNP is not an option; you would shop Anthem's general HMO, HMO-POS or PPO plans instead.
HMO, HMO-POS or PPO — what the plan type actually means
Anthem's lineup here spans three plan types, and the type matters more than the plan name:
- HMO plans generally require you to use in-network doctors and hospitals and to pick a primary care doctor who coordinates referrals.
- HMO-POS plans work like an HMO but add a limited "point-of-service" option to see some out-of-network providers, usually at a higher cost.
- PPO plans let you see out-of-network providers, usually at a higher cost share, without a referral. A regional PPO uses a CMS-defined service area.
Every Anthem Medicare Advantage plan in the county for 2027 includes Part D prescription drug coverage, so you would not add a separate standalone Part D plan.
How to check a plan against your doctors and drugs before December 7
Before you keep, join or switch any plan, do this short check for each one you are considering. Nothing here replaces the plan's own documents, but it will catch most problems.
1. Start at medicare.gov and open the Plan Finder. Enter your ZIP code and your prescription drug list exactly as written on your bottles, including dose and quantity. 2. Add your pharmacies. The same drug can cost very different amounts at different pharmacies on the same plan. 3. For each plan you are considering, look up each of your doctors, specialists and hospitals in that plan's provider directory for the 2027 plan year — not this year's. Networks can change on January 1. 4. If any drug or doctor is important to you, call the plan directly using the number on medicare.gov to confirm 2027 coverage. Networks and formularies that show on a public page can change up until the plan year starts. 5. Read your Annual Notice of Change if you already have an Anthem plan. It spells out what is different about your plan next year.

What is changing in Medicare itself for 2027
A few program-level numbers are already set for 2027 and apply no matter which plan or carrier you choose:
- The Part D out-of-pocket cap rises to $2,400 in 2027 (it is $2,100 in 2026).
- The Part D maximum deductible rises to $700 in 2027 (it is $615 in 2026).
- The Part D national base beneficiary premium for 2027 is $41.33 — that is a formula input CMS uses, not what any specific plan charges.
- The coverage gap, or "donut hole," has been gone since 2025; Part D now has three phases.
- The Medicare Prescription Payment Plan remains available if you want to spread your drug costs across the year in level monthly payments instead of paying at the pharmacy counter.
The 2027 Part B standard premium and deductible, along with the 2027 IRMAA income brackets, have not yet been announced. CMS typically publishes those figures later in the fall.
